目的 探讨餐前饮水疗法对老年餐后低血压(PPH)患者血压波动和心脑血管事件的影响,为非药物治疗PPH提供循证依据.方法 采用随机对照试验设计,纳入300例老年PPH患者,随机分为观察组(常规治疗+餐前30 min饮用400 mL温开水)和对照组(常规治疗),每组150例.比较2组患者治疗前和治疗6个月后三餐前及三餐后2 h内收缩压和舒张压的最低值,记录治疗6个月内心脑血管事件的发生情况.采用Kaplan-Meier生存曲线评估干预效果.结果 治疗前,2组患者收缩压、舒张压基线水平的差异无统计学意义(均P>0.05).治疗6个月后,观察组三餐后2 h内收缩压波动幅度显著低于对照组(P<0.001),而2组间舒张压的差异无统计学意义(P>0.05).治疗6个月内,观察组总心脑血管事件发生率显著低于对照组(P=0.014 5).结论 餐前饮水疗法可有效降低老年PPH患者餐后收缩压波动幅度,减少心脑血管事件发生风险,安全性良好,可作为临床非药物治疗方案.

Objective To investigate the effects of pre-meal water intake therapy on blood pressure(BP)fluctuations and cardiovascular and cerebrovascular events in older patients with postprandial hypotension(PPH)and to provide evidence for non-pharmacological mana-gement of PPH.Methods In this randomized controlled trial,300 older patients with PPH were randomly assigned to an observation group(routine treatment plus 400 mL warm water 30 min before meals)and control group(routine treatment alone),with 150 patients in each group.The lowest systolic(SBP)and diastolic blood pressure(DBP)values before daily three meals and within 2 h after each meal were compared between the two groups before treatment as well as 6 months after treatment.Cardiovascular and cerebrovascular events were documented during the 6-month follow-up period.Kaplan-Meier survival analysis was performed to evaluate the intervention effect.Results No statistically significant differences were observed in baseline SBP or DBP between the two groups(both P>0.05)before treatment.The observation group showed significantly reduced postprandial SBP fluctuations 6 months after treatment compared with those in the control group(P<0.001),whereas no significant difference was observed in DBP(P>0.05).During the 6-month follow-up period,the total incidence of cardiovascular and cerebrovascular events was significantly lower in the observation group than in the control group(P=0.014 5).Conclusion Pre-meal water intake therapy effectively reduces postprandial SBP fluctuations and lowers the risk of cardiovascular and cerebrovascular events in older patients with PPH,showing favorable safety.Therefore,it can be recommended as a non-pharmacological treatment option in clinical practice.
