HER2零表达与低表达乳腺癌的临床特征和新辅助治疗效果分析OA
Clinical characteristics and neoadjuvant treatment efficacy of HER2 zero-expression and low-expression breast cancer
目的 探讨人表皮生长因子受体2(HER2)零表达(HER2-0)与HER2低表达乳腺癌患者的临床特征以及新辅助治疗效果的差异.方法 收集2016年1月至2020年12月中国医科大学附属第一医院乳腺外科收治的303例HER2-0与HER2低表达乳腺癌患者的临床资料,主要研究终点是乳腺病理完全缓解(bpCR).采用t检验和χ2检验比较HER2-0与HER2低表达乳腺癌患者各项临床指标的差异.采用logistic单因素和多因素回归分析,并结合基于倾向评分的治疗加权逆概率(IPTW)比较HER2-0与HER2低表达乳腺癌患者,以及不同激素受体(HR)表达患者间新辅助治疗效果的差异.结果 HER2-0与HER2低表达患者确诊时的年龄、绝经状态、生育史、锁骨淋巴结侵犯、胸肌腋窝淋巴结侵犯、术前临床T分期和N分期、TNM分期和手术方式比较均无统计学差异(均P>0.05).HER2低表达和HER2-0患者新辅助治疗后bpCR比较无统计学差异(P>0.05).在HR阴性患者中,HER2低表达患者新辅助治疗后bpCR显著优于HER2-0患者(P<0.05);而在HR阳性患者中,HER2低表达与HER2-0患者的bpCR比较无统计学差异(P>0.05).结论 HER2-0与HER2低表达乳腺癌患者的临床特征未见明显差异.HR阴性患者中HER2低表达患者新辅助治疗效果显著优于HER2-0患者.不同HR表达水平的HER2低表达乳腺癌可能是一种独特的亚型,值得更多关注.
Objective To explore the differences in clinical characteristics and neoadjuvant treatment outcomes between patients with human epidermal growth factor receptor 2(HER2)zero-expression(HER2-0)and HER2-low expression breast cancer.Methods Cli-nical data from 303 patients with HER2-0 and HER2-low breast cancer treated at the Department of Breast Surgery,the First Hospital of China Medical University,were collected from January 2016 to December 2020.The primary study endpoint was breast pathological complete response(bpCR).The t and χ2 test were used to compare the differences in clinical indicators between HER2-0 and HER2-low breast cancer patients.Univariate and multivariate logistic regression analyses were performed,combined with inverse probability of treat-ment weighting(IPTW)based on propensity scores,to compare differences in neoadjuvant therapy efficacy between patients with HER2-0 and HER2-low breast cancer and among patients with different hormone receptor(HR)expression statuses.Results No statistically significant differences were observed between HER2-0 and HER2-low patients in terms of age at diagnosis,menopausal status,reproduc-tive history,supraclavicular lymph node involvement,pectoral axillary lymph node involvement,preoperative clinical tumor(T)and node(N)stages;tumor,node,and metastasis(TNM)stage;and surgical approach(all P>0.05).No statistically significant differences in bpCR after neoadjuvant therapy were observed between HER2-low and HER2-0 patients(P>0.05).Among HR-negative patients,HER2-low patients showed significantly better bpCR after neoadjuvant therapy compared with HER2-0 patients(P<0.05).However,among HR-positive patients,no statistically significant differences in bpCR were observed between HER2-low and HER2-0 patients(P>0.05).Conclusion No significant differences in clinical characteristics were observed between HER2-0 and HER2-low breast cancer patients.The neoadjuvant treatment effect of HER2 low-expression patients is significantly better than that of HER2-0 patients among HR-negative patients.Therefore,HER2-low breast cancer with different HR expression levels may represent a distinct subtype that warrants further study.
