低剂量对比剂联合宝石能谱CT在头颈部CTA检查中的应用价值及对碘负荷与辐射剂量的影响OA
Application Value of Low-dose Contrast Medium Combined with Gemstone Spectral CT in Head and Neck CTA Examination and Its Impact on Iodine Load and Radiation Dose
目的:探讨低剂量对比剂联合宝石能谱 CT 在头颈部 CT 血管造影(computed tomography angiography,CTA)检查中的应用价值及对碘负荷与辐射剂量的影响.方法:前瞻性选择 2024 年 1 月—2025 年 6 月抚州市第一人民医院接诊的疑似头颈部动脉疾病高危患者 90 例为对象,以随机数字表法分为两组,各 45 例.两组均完成头颈部 CTA 检查,A 组采用常规剂量对比剂联合 120 kV 混合能量 CT 检查,B 组采用低剂量对比剂联合宝石能谱 CT 60 keV 检查.比较两组碘负荷、辐射剂量、图像质量.结果:两组主动脉弓、颈总动脉、颈内动脉 C1 段、大脑中动脉 M1 段的 CT 值、对比度噪声比(contrast-to-noise ratio,CNR)及信噪比(signal-to-noise ratio,SNR)比较,差异无统计学意义(P>0.05).两组主动脉弓、颈总动脉、颈内动脉及大脑中动脉主观图像质量评分比较,差异无统计学意义(P>0.05).B 组剂量长度乘积(dose-length product,DLP)、有效辐射剂量(effective dose,ED)低于 A 组,对比剂用量及碘用量少于 A 组(P<0.05).结论:低剂量对比剂联合宝石能谱 CT 用于头颈部 CTA 检查中效果良好,在满足图像诊断要求的同时,能降低碘负荷及辐射剂量.
Objective:To investigate the application value of low-dose contrast medium combined with gemstone spectral CT in head and neck CT angiography(CTA)and its impact on iodine load and radiation dose.Method:Prospectively to selection was made of 90 patients diagnosed with high-risk head and neck arterial diseases who were treated at the First People's Hospital of Fuzhou City from January 2024 to June 2025.These patients were divided into two groups based on random number table method,with 45 cases in each group.Both groups completed head and neck CTA examination.Group A received a conventional dose contrast medium with 120 kVp polychromatic CT examination,while group B received low-dose contrast medium with 60 keV gemstone spectral CT examination.The two groups were compared in terms of iodine load,radiation dose,and image quality.Result:There were no statistically significant differences in the CT values,contrast-to-noise ratio(CNR),and signal-to-noise ratio(SNR)of aortic arch,common carotid artery,C1 segment of internal carotid artery and M1 segment of middle cerebral artery between the two groups(P>0.05).The comparison of subjective image quality scores of aortic arch,common carotid artery,internal carotid artery and middle cerebral artery in the two groups showed no statistically significant difference(P>0.05).The dose-length product(DLP),effective dose(ED)in group B were lower than those in group A,contrast medium dosage and iodine dosage were less than those in group A(P<0.05).Conclusion:Low-dose contrast medium combined with gemstone spectral CT is effective for head and neck CTA examination.It can meet the requirements of image diagnosis while reducing iodine load and radiation dose.
