目的 基于孟德尔随机化(MR)方法明确肾功能与骨密度的关系来佐证中医肾主骨理论的科学性.方法 采用孟德尔随机化研究方法,基于东亚人群的全基因组关联研究(GWAS)汇总数据,提取143 658例估算肾小球滤过率(eGFR)样本及212 453例骨质疏松症(OP)样本数据.选取与eGFR高度相关的单核苷酸多态性(SNPs)作为工具变量,以逆方差加权法(IVW)为主要方法,辅助MR-Egger回归及加权中位数法、加强模式、普通模式进行孟德尔随机化分析,评估eGFR对OP风险的影响程度.使用SMR方法鉴定eGFR在骨骼肌肉上的药物靶点.结果 共鉴定出141个SNP作为合适的工具变量.MR-Egger回归未显示存在显著的水平多效性证据(P=0.061).IVW分析显示,较低的eGFR与OP风险增加存在关联,具体而言,对数转换后的eGFR每降低一个标准差,OP风险增加24.8%(P=0.001,OR=1.248,95%CI:1.093~1.425).发现了多个通过干预eGFR治疗骨骼肌肉相关疾病的靶点.结论 本研究从遗传学角度佐证了中医理论肾主骨的科学性,确认了遗传学预测的肾功能减退是骨质疏松症的危险因素.

Objective To clarify relationship between renal function and bone mineral density based on Mendelian ran-domization(MR)method to verify the scientific validity of the traditional Chinese medicine theory that"the kidney governs the bones".Methods MR method was adopted,data from 143 658 estimated glomerular filtration rate(eGFR)samples and 212 453 osteoporosis(OP)samples were extracted based on genome-wide association studies(GWAS)of East Asian populations.Single nucleotide polymorphisms(SNPs)strongly associated with eGFR were selected as instrumental varia-bles.The inverse variance weighted method(IVW)was used as the main approach,assisted by MR Egger regression and weighted median method,enhanced mode,and normal mode for MR analysis,the impact of eGFR on OP risk was evaluated.SMR method was used to identify drug targets of eGFR in skeletal muscles.Results A total of 141 SNPs were identified as suitable instrumental variables.MR-Egger regression did not show significant evidence of horizontal pleiotropy(P=0.061).IVW analysis showed a correlation between lower eGFR and increased risk of osteoporosis.Specifically,for every one stand-ard deviation decrease in eGFR after logarithmic transformation,the risk of osteoporosis increased by 24.8%(P=0.001,OR=1.248,95%CI:1.093-1.425).Multiple targets have been identified for the treatment of musculoskeletal disorders through intervention in eGFR.Conclusion This study provides genetic evidence supporting the scientific validity of the tra-ditional Chinese medicine theory that"the kidney governs bones",and confirms that genetically predicted impaired kidney function is a risk factor for osteoporosis.
