GA/HbA1c、LCR、BAR在不同严重程度1型糖尿病酮症酸中毒患儿血清中的表达及意义OA
Expression and significance of GA/HbA1c,LCR and BAR in the serum of children with type 1 diabetic ketoacidosis of different severity
目的 探讨血清糖化白蛋白/糖化血红蛋白比值(GA/HbA1c)、淋巴细胞与C反应蛋白比值(LCR)、胆红素与白蛋白比值(BAR)在不同严重程度1型糖尿病酮症酸中毒(T1DM-DKA)患儿中的表达及临床意义.方法 选取2022年10月-2025年10月四川省简阳市人民医院儿科收治的T1DM-DKA患儿84例作为DKA组,单纯T1DM患儿84例作为T1DM组,另选取医院同期健康体检儿童84例作为健康对照组.根据不同病情程度,将T1DM-DKA患儿分为轻度亚组29例、中度亚组32例、重度亚组23例.采用Pearson相关性分析GA/HbA1c、LCR、BAR与T1DM-DKA患儿病情严重程度的相关性;采用受试者工作特征(ROC)曲线分析GA/HbA1c、LCR、BAR对重度T1DM-DKA患儿的诊断价值,通过DCA曲线分析GA/HbA1c、LCR、BAR联合对评估重度T1DM-DKA的临床净获益.结果 3组GA/HbA1c、BAR比较,DKA组>T1DM组>健康对照组,而LCR比较,DKA组<T1DM组<健康对照组(F/P=262.599/<0.001、329.842/<0.001、217.262/<0.001);3 亚组 GA/HbA1c、BAR 比较,重度亚组>中度亚组>轻度亚组,而LCR 比较,重度亚组<中度亚组<轻度亚组(F/P=77.044/<0.001、103.172/<0.001、24.369/<0.001);Pearson 相关性分析显示,T1DM-DKA患儿GA/HbA1c、BAR与病情严重程度呈正相关(r=0.625/<0.001、0.521/<0.001),LCR与病情严重程度呈负相关(r/P=-0.595/<0.001);GA/HbA1c、LCR、BAR单独及联合诊断重度T1DM-DKA患儿的曲线下面积(AUC)分别为 0.790、0.783、0.747、0.882,三者联合优于各自单独诊断(Z/P=2.102/<0.001、2.122/<0.001、2.991/<0.001);DCA曲线分析结果表明,当T1DM-DKA患儿病情严重程度的风险阈值为0.08~0.88时,GA/HbA1c、LCR、BAR联合检测的临床净获益大于3种指标单独检测.结论 GA/HbA1c、BAR随T1DM-DKA患儿病情加重而升高,LCR随病情加重而降低,三者联合检测对重度T1DM-DKA患儿具有较高诊断价值.
Objective To investigate the expression and clinical significance of serum glycated albumin/glycated he-moglobin ratio(GA/HbA1c),lymphocyte-to-C-reactive protein ratio(LCR),and bilirubin-to-albumin ratio(BAR)in children with type 1 diabetes mellitus complicated by diabetic ketoacidosis(T1DM combined with DKA)of different severities.Meth-ods A total of 84 children with T1DM combined with DKA admitted to the Department of Pediatrics,Jianyang People's Hospital of Sichuan Province from October 2022 to October 2025 were enrolled as the DKA group,84 children with simple T1DM as the T1DM group,and 84 healthy children undergoing physical examination during the same period as the healthy control group.Children with T1DM combined with DKA were further divided into a mild subgroup(n=29),a moderate sub-group(n=32),and a severe subgroup(n=23).Pearson correlation analysis was used to evaluate the correlation between serum GA/HbA1c,LCR,BAR levels and the severity of T1DM-DKA.ROC curve analysis was performed to assess the diagnostic value of each index for severe T1DM-DKA,and decision curve analysis(DCA)was used to evaluate the clinical net benefit of diagnosing severe T1DM-DKA.Results Serum GA/HbA1c and BAR levels:DKA group>T1DM group>healthy control group.Serum LCR levels:DKA group<T1DM group<healthy control group(F/P=262.599/<0.001,217.262/<0.001,329.842/<0.001).Serum GA/HbA1c and BAR levels:severe subgroup>moderate subgroup>mild subgroup.Serum LCR lev-els:severe subgroup<moderate subgroup<mild subgroup(F/P=262.599/<0.001,329.842/<0.001,217.262/<0.001).Correla-tion analysis showed that serum GA/HbA1c(r=0.625)and BAR(r=0.521)levels were positively correlated with the severity of DKA,while LCR level(r=-0.595)was negatively correlated with the severity of DKA(all P<0.001).The AUCs of serum GA/HbA1c,LCR,BAR,and their combination for diagnosing severe T1DM-DKA in children were 0.790,0.783,0.747,and 0.882,respectively.The combined AUC was superior to each individual marker(Z/P=2.102/<0.001,2.122/<0.001,2.991/<0.001).DCA curve analysis indicated that when the risk threshold for disease severity in children with T1DM-DKA was 0.08-0.88,the clinical net benefit of combined detection of serum GA/HbA1c,LCR,and BAR was greater than that of each marker alone.Conclusion Serum GA/HbA1c and BAR levels increase with the aggravation of T1DM-DKA in children,while LCR level decreases.Combined detection of the three indicators has high diagnostic value for severe T1DM-DKA.
