Moxibustion regulates the Keap1/Nrf2 pathway to inhibit ferroptosis and ameliorate intestinal inflammation in mice with Crohn's disease

目的 基于Kelch样ECH相关蛋白 1(Keap1)/核因子E2 相关因子 2(Nrf2)通路探讨艾灸对克罗恩病(CD)小鼠肠道铁死亡的调控作用及其可能的作用机制.方法 选取 30 只SPF级雄性C57BL/6 小鼠,随机分为正常组、模型组及艾灸组,每组 10 只.模型组与艾灸组通过自由饮用 2%葡聚糖硫酸钠盐(DSS)溶液连续 14 d建立CD模型.艾灸组于造模后对双侧天枢行悬灸治疗,每日 1 次,每次 30 min,连续干预 14 d.正常组与模型组不予艾灸处理.干预结束后,观察各组小鼠的一般情况、疾病活动指数(DAI)评分及结肠组织病理变化;采用ELISA法检测血清炎症因子肿瘤坏死因子(TNF)-α、白细胞介素(IL)-1β、IL-6、IL-17 水平及结肠组织氧化应激与铁死亡相关指标超氧化物歧化酶(SOD)、谷胱甘肽(GSH)、谷胱甘肽过氧化物酶 4(GPX4)、丙二醛(MDA)、亚铁离子(Fe2+)水平;通过免疫荧光染色检测结肠组织活性氧(ROS)表达;应用Western blot检测结肠组织样本中Keap1、Nrf2、血红素加氧酶(HO)-1、醌氧化还原酶 1(NQO1)的蛋白表达情况.结果 与正常组比较,模型组小鼠DAI评分显著升高(P<0.01),结肠组织出现明显炎性细胞浸润、隐窝结构破坏;血清炎症因子TNF-α、IL-1β、IL-6、IL-17 水平均明显上升(P<0.01);结肠组织SOD活性、GSH、GPX4 含量均明显下降(P<0.01),而MDA、Fe2+含量及ROS水平均明显上升(P<0.01);同时,Keap1 蛋白表达明显上调(P<0.01),Nrf2、HO-1、NQO1 蛋白表达则明显下调(P<0.01).与模型组相比,艾灸组小鼠DAI评分显著降低(P<0.01),结肠组织病理损伤明显减轻;血清炎症因子TNF-α、IL-1β、IL-6、IL-17 水平均显著下降(P<0.01 或P<0.05);结肠组织SOD活性、GSH、GPX4 含量均显著回升(P<0.01 或P<0.05),MDA、Fe2+含量及ROS各指标水平均明显下降,差异具有统计学意义(P<0.05 或P<0.01);Keap1 蛋白的表达水平明显下调(P<0.01),而Nrf2、HO-1 与NQO1 的蛋白表达均显著上调(P<0.01).结论 艾灸天枢穴可改善克罗恩病小鼠肠道炎症损伤,其作用机制与调控 Keap1/Nrf2 通路、增强抗氧化能力、抑制肠道铁死亡密切相关.

Objective To investigate the regulatory effects of moxibustion on intestinal ferroptosis in mice with Crohn's disease(CD)based on the Kelch-like ECH-associated protein 1(Keap1)/nuclear factor erythroid 2-related factor 2(Nrf2)pathway and its possible mechanism of action.Methods Thirty SPF-grade male C57BL/6 mice were randomly divided into normal group,model group,and moxibustion group,with 10 mice in each group.The model group and moxibustion group were given free access to 2%dextran sulfate sodium(DSS)solution for 14 consecutive days to establish CD models.After modeling,the moxibustion group received suspended moxibustion at bilateral"Tianshu"(ST25)once daily for 30 minutes each time for 14 consecutive days.No moxibustion was performed in the normal group and model group.After the intervention,general conditions,disease activity index(DAI)scores,and colon histopathological changes were observed in each group of mice.ELISA was used to determine the levels of serum inflammatory factors including tumor necrosis factor(TNF)-α,interleukin(IL)-1β,IL-6,and IL-17,as well as colonic oxidative stress and ferroptosis-related indicators such as superoxide dismutase(SOD),glutathione(GSH),glutathione peroxidase 4(GPX4),malondialdehyde(MDA),and ferrous ion(Fe2+).Reactive oxygen species(ROS)expression in colon tissue was measured by immunofluorescence staining.The protein expression levels of Keap1,Nrf2,heme oxygenase(HO)-1,and NAD(P)H:quinone oxidoreductase 1(NQO1)in colon tissue samples were determined by Western blot.Results Compared with the normal group,the mice in the model group showed a significant increase in DAI scores(P<0.01),marked inflammatory cell infiltration,and crypt structure destruction in colon tissue.The levels of serum inflammatory factors,including TNF-α,IL-1β,IL-6,and IL-17,were significantly elevated(P<0.01).SOD activity and levels of GSH and GPX4 in colon tissue were significantly decreased(P<0.01),while levels of MDA,Fe2+,and ROS were significantly increased(P<0.01).Meanwhile,Keap1 protein expression was significantly upregulated(P<0.01),whereas protein expression of Nrf2,HO-1,and NQO1 was significantly downregulated(P<0.01).Compared with the model group,the mice in the moxibustion group showed a significant reduction in DAI score(P<0.01)and marked alleviation of pathological damage in colon tissue.The levels of serum inflammatory factors,including TNF-α,IL-1β,IL-6,and IL-17,were significantly reduced(P<0.01 or P<0.05).SOD activity and levels of GSH and GPX4 in colon tissue were significantly increased(P<0.01 or P<0.05),while levels of MDA,Fe2+,and ROS were significantly decreased,and the differences were statistically significant(P<0.05 or P<0.01).Keap1 protein expression level was significantly downregulated(P<0.01),while protein expression of Nrf2,HO-1,and NQO1 was significantly upregulated(P<0.01).Conclusion Moxibustion at"Tianshu"(ST25)can ameliorate intestinal inflammatory injury in mice with CD,and its mechanism of action is closely related to regulating the Keap1/Nrf2 pathway,enhancing antioxidant capacity,and inhibiting intestinal ferroptosis.
