缺血性脑卒中是全球范围内具有高发病率、高致残率与高致死率的重大脑血管疾病,颅内动脉粥样硬化(ICAS)则是其核心病理生理学基础.高分辨率磁共振血管壁成像(HR-VWI)能够直观显示血管壁病变,该技术与影像组学相结合,能够定量挖掘斑块微观影像学特征,为ICAS 斑块的精准风险分层、缺血性脑卒中早期预警及个体化诊疗策略制定提供了全新技术手段.本文围绕HR-VWI联合影像组学技术,系统综述其在评估ICAS斑块易损性、解析斑块影像学特征与缺血性脑血管事件的内在关联、辅助临床诊疗决策等方面的最新研究进展及临床应用价值.

Ischemic stroke is a major cerebrovascular disease with high incidence,high disability rate and high mortality worldwide.Intracranial atherosclerotic(ICAS)is the core pathological basis of this disease.High-resolution magnetic resonance vessel wall imaging(HR-VWI)can directly display vascular wall lesions.This technology combined with radiomics can quantitatively explore the microscopic imaging features of plaques,providing a new technical means for precise risk stratification of ICAS plaques,early warning of ischemic stroke and formulation of individualized treatment strategies.This article focuses on HR-VWI combined with radiomics technology,systematically reviewing the latest research progress and clinical application value in evaluating the vulnerability of ICAS plaques,analyzing the intrinsic association between plaque imaging features and ischemic cerebrovascular events,and assisting clinical diagnosis and treatment decisions.
