目的 探讨组织多普勒成像(TDI)联合心肌声学造影(MCE)对经皮冠状动脉介入治疗(PCI)后心肌微循环的预测价值.方法 回顾性采集 2023 年 3 月至 2025 年 3 月在湖州市第一人民医院完成 PCI 治疗的 93 例心肌梗死患者资料,所有患者 PCI 后 12 h 内均接受 TDI、MCE 检查,术后 1 周接受冠状动脉造影,通过医院电子病历系统获得患者完整资料.根据微循环阻力指数评估PCI术后心肌微循环情况,并分为发生组26例和未发生组67例.比较两组一般资料、术后12 h内TDI定量参数[侧壁与室间隔侧收缩期峰值速度(Vs与Vs')、早期舒张峰值速度(Ve与 Ve')、晚期舒张峰值速度(Va 与 Va'),并计算 Ve/Va、Tei]、MCE 定量参数[曲线平台期峰值强度(A 值)、曲线上升斜率(β 值)、心肌血流量(A×β 值)]的差异.采用多因素 Logistic 回归分析筛选 PCI 后发生心肌微循环障碍的影响因素,绘制 ROC 曲线分析 TDI、MCE 参数单独及联合应用对行PCI的患者术后发生心肌微循环障碍的预测效能.结果 发生组Tei高于未发生组,Ve'、Ve、Ve/Va、A值、β值、A×β值均低于未发生组(t=2.807、5.892、4.242、3.231、3.106、2.806、2.285,P<0.05);经 Logistic 回归分析显示,Tei、Ve'、Ve、Ve/Va、A 值、β 值、A×β 值均为 PCI 后发生心肌微循环障碍的影响因素,各参数单独及联合预测 PCI 后患者心肌微循环障碍的 AUC 分别为 0.675、0.803、0.723、0.687、0.692、0.671、0.644、0.933,联合预测的价值更高.结论 TDI联合MCE可较好地预测行PCI后患者心肌微循环障碍情况,具有较高的临床价值.

Objective To explore the value of tissue Doppler imaging(TDI)combined with myocardial contrast echocardiography(MCE)in predicting myocardial microcirculation after percutaneous coronary intervention(PCI).Methods Data of 93 patients with myocardial infarction who underwent PCI treatment at the First People's Hospital of Huzhou from March 2023 to March 2025 were collected.All patients received TDI and MCE examinations within 12 hours after PCI,and coronary angiography was performed one week after the operation.Complete patient data were obtained through the hospital's electronic medical record system.Postoperative myocardial microcirculation was evaluated based on the microcirculation resistance index,and patients were divided into the occurrence group(26 cases)and the non-occurrence group(67 cases).The general data,TDI quantitative parameters within 12 hours after the operation[peak systolic velocity of the lateral wall(Vs)and of the interventricular septum(Vs'),early diastolic peak velocity of the lateral wall(Ve)and of the septum(Ve'),late diastolic peak velocity of the lateral wall(Va)and of the septum(Va'),the calculated Ve/Va ratios(for both lateral wall and septum),and the Tei index],as well as MCE quantitative parameters[peak intensity of the plateau phase(A value),slope of curve rise(β value),and myocardial blood flow(A×β value)]were compared between the two groups.Multivariate Logistic regression analysis was used to screen influencing factors for myocardial microcirculation dysfunction after PCI.ROC curve was drawn to analyze the predictive efficacy of TDI and MCE parameters alone and in combination for postoperative myocardial microcirculation dysfunction in patients undergoing PCI.Results The Tei index was higher,while Ve',Ve,Ve/Va,A value,β value,and A×β value were all lower in the occurrence group than in the non-occurrence group(t=2.807,5.892,4.242,3.231,3.106,2.806,2.285;all P<0.05).Logistic regression analysis showed that Tei,Ve',Ve,Ve/Va,A value,β value,and A×β value were all influencing factors for the occurrence of myocardial microcirculation dysfunction after PCI.The AUC of each parameter alone and in combination in predicting myocardial microcirculation dysfunction in patients after PCI was 0.675,0.803,0.723,0.687,0.692,0.671,0.644,and 0.933,respectively.The value of combined prediction was higher.Conclusion TDI combined with MCE can effectively predict myocardial microcirculation dysfunction in patients after PCI and has high clinical value.
