基于"虚气留滞"理论探讨脾瘅方运用扶正祛邪法治疗糖尿病前期OA

Prediabetes,a critical transitional stage to type 2 diabetes,aligns closely with the traditional Chinese medi-cine(TCM)theory of"Stagnation of deficient Qi".This study systematically explored the theoretical and clinical applica-tion of Pidan Prescription(脾瘅方)in treating prediabetes through the TCM approach of"supporting healthy qi and elimi-nating pathogenic factors".Rooted in classical texts such as Suwen and Treatise on the Spleen and Stomach,the pathogene-sis of Pidan Prescription is attributed to spleen deficiency leading to stagnation of Qi,blood and body fluids,resulting in phlegm-dampness,blood stasis and heat accumulation.Pidan Prescription employs bran-fried Cangzhu(Atractylodis Rhi-zoma)and Baizhu(Atractylodis Macrocephalae Rhizoma)as monarch herbs to strengthen spleen and resolve dampness,com-plemented by Huanglian(Coptidis Rhizoma)and Zhizi(Gardeniae Fructus)to clear heat,Gualou(Trichosanthis Fructus)and Zhishi(Aurantii Fructus Immaturus)to dispel phlegm-stagnation,and Guijianyu(Ramulus Euonymi)to activate blood circu-lation.This formula embodies the TCM principle of"restoring Qi transformation and resolving stagnation"through harmo-nizing the liver-spleen axis,regulating Qi movement and eliminating pathological products.Modern research confirms its multi-target effects,including improving insulin resistance via MAPK/AMPK pathways and modulating gut microbiota,while alleviating fatigue and obesity.Compared to Western medicine single-target approach,Pidan Formula integrates nat-ural components with holistic regulation,offering a safer and more comprehensive TCM strategy for prediabetes manage-ment.
