基于中医心身医学思想探讨疼痛的证治思路OA

Pain is a complex emotional experience involving unpleasant sensory and emotional perception,arising not only from physiological injury but also from closely intertwined psychological and social factors.Traditional Chinese medi-cine embodies a rich tradition of psychosomatic thought,with the Huangdi Neijing explicitly stating that"all pains pertain to the Heart."Based on the Neijing,this paper explores the mechanisms of pain from three perspectives—the Heart gov-erning spirit,the Heart governing blood and vessels,and the Heart as a Yang organ—and analyzes how pain development is closely associated with the perceptual role of Heart-spirit,the circulation of blood in the vessels,and the action of Heart-fire.Accordingly,it proposes that the clinical management of pain disorders should focus on the Heart by selecting Heart-meridian herbs in prescriptions,using acupoints related to the Heart system in acupuncture to regulate spirit,and incorporating psychological and emotional therapies,thereby integrating physical and mental approaches.In light of the 2020 revised definition of pain by the International Association for the Study of Pain,this paper emphasizes that,beyond treating the primary condition,greater attention should be paid to patients'emotional and psychological factors,and that multimodal therapies should be employed to enhance analgesic efficacy,thereby offering new perspectives for the clinical management of pain disorders.
