目的 调查食管癌组织中溶酶体相关膜蛋白 2A 型(LAMP2a)、富含亮氨酸的 α-2-糖蛋白 1(LRG1)表达,并分析其对预后的预测价值.方法 前瞻性选取 2022 年 3 月至 2024 年 3 月濮阳市人民医院诊治的 112 例食管癌患者为研究对象,均于术中切除癌组织及癌旁组织,比较其 LAMP2a、LRG1 蛋白表达情况.分析不同临床特征患者 LAMP2a、LRG1 蛋白表达情况,并探讨其与临床特征相关性.术后随访 1 a,并将患者分为预后不良亚组和预后良好亚组,比较 2 个亚组 LAMP2a、LRG1 蛋白表达情况.采用 Cox 回归分析预后的影响因素,并进一步分析 LAMP2a、LRG1 在预后不良风险中的交互作用,并评价其对预后的预测价值.结果 癌组织中 LAMP2a、LRG1蛋白阳性表达率高于癌旁组织(P<0.05);LAMP2a、LRG1 蛋白阳性表达与 TNM 分期、淋巴结转移呈正相关,而与分化程度呈负相关(P<0.05);预后不良亚组 LAMP2a、LRG1 蛋白阳性表达率高于预后良好亚组(P<0.05);TNM分期、分化程度、淋巴结转移及 LAMP2a、LRG1 蛋白阳性表达率为预后的独立影响因素(P<0.05);LAMP2a、LRG1蛋白阳性表达时预后不良风险的 OR 为 7.023,交互作用指数为 1.405,且 LAMP2a、LRG1 蛋白阳性表达在预后不良风险中呈正向交互作用;LAMP2a、LRG1 阳性表达联合预测预后的 AUC 值显著高于单一指标预测(P<0.05).结论 食管癌组织中 LAMP2a、LRG1 蛋白阳性表达与临床特征、预后密切相关,且 LAMP2a、LRG1 蛋白阳性表达在预后不良风险中呈正向交互作用,联合检测二者水平对患者预后的预测价值更高.

Objective To investigate the expression of lysosomal-associated membrane protein 2A(LAMP2a)and leucine-rich α-2-glycoprotein 1(LRG1)in esophageal cancer tissues,and to analyze their predictive value for prognosis.Methods A prospective study was conducted on 112 patients with esopha-geal cancer who were diagnosed and treated at Puyang People's Hospital hospital from March 2022 to March 2024.The cancer tissues and adjacent tissues were removed during the operation.The expressions of LAMP2a and LRG1 proteins were compared.The expressions of LAMP2a and LRG1 proteins in patients with different clinical characteristics was analyzed,and its correlation with clinical characteristics was discussed.The pa-tients were followed up for 1 a,and were divided into poor prognosis subgroup and good prognosis subgroup;the expressions of LAMP2a and LRG1 proteins in the two subgroups were compared.Cox regression was used to analyze the prognostic factors.The interaction between LAMP2a and LRG1 in the risk of poor prognosis was further analyzed,and its predictive value for prognosis was evaluated.Results The positive expression rates of LAMP2a and LRG1 protein in cancer tissues were higher than those in adjacent tissues(P<0.05).The positive expression of LAMP2a and LRG1 protein was positively correlated with TNM stage and lymph node metastasis,but negatively correlated with the degree of differentiation(P<0.05).The positive expression rates of LAMP2a and LRG1 protein in the poor prognosis subgroup were higher than those in the good prognosis subgroup(P<0.05).TNM stage,degree of differentia-tion,lymph node metastasis and positive expression of LAMP2a and LRG1 protein were independent prognostic factors(P<0.05).The OR of LAMP2a and LRG1 protein positive expression in the risk of poor prognosis was 7.023,and the interaction index was 1.405.The positive expression of LAMP2a and LRG1 protein showed a positive interaction in the risk of poor prognosis.The AUC value of combined prediction of LAMP2a and LRG1 positive expression was significantly higher than that of single index prediction(P<0.05).Conclusion The positive expression of LAMP2a and LRG1 protein in esophageal cancer tissues is closely related to clinical characteristics and prognosis,and the positive expression of LAMP2a and LRG1 protein has a positive interaction in the risk of poor prognosis.The combined detection of their levels has a higher predictive value for patient prognosis.
