体面劳动视域下医疗护理员角色清晰度对留职意愿的影响OA

目的:探讨医疗护理员角色清晰度对留职意愿的影响机制,并分析职业认同的中介作用及体面劳动感在其中的调节效应.方法:采用便利抽样法,于2025年8月—10月选取广东省8所无陪照护服务试点医疗机构的473名医疗护理员作为研究对象.采用一般资料调查表、角色清晰度量表、职业认同评定量表、体面劳动感知量表及留职意愿量表进行调查.应用SPSS 27.0软件进行相关性分析、中介效应分析和调节效应分析.结果:医疗护理员留职意愿得分为13.00(12.00,16.00)分,年龄、文化程度、每日工作时间及聘用关系对医疗护理员留职意愿有影响(P<0.05).角色清晰度、职业认同、体面劳动感知与留职意愿均呈正相关(P<0.01).职业认同在角色清晰度与留职意愿间发挥部分中介作用,间接效应值为0.084(占总效应的39.25%).体面劳动感知对角色清晰度→职业认同路径存在负向交互调节作用(效应值=-0.047,P<0.001),且在低体面劳动感知情境下角色清晰度对职业认同的边际贡献更明显.结论:角色清晰度不仅能直接正向影响医疗护理员的留职意愿,还能通过职业认同的间接作用增强留职意愿.体面劳动感知具有调节效应,在低体面劳动情境下,角色清晰度对职业认同的保护作用更明显.

Objective:To explore the mechanism through which role clarity influences the intent to stay among medical nursing assistants,and to examine the mediating role of professional identity and the moderating effect of decent work perception.Methods:A convenience sampling method was used to select 473 medical nursing assistants from eight pilot medical institutions of"accompanied-free care"services in Guangdong province between August and October 2025.Data were collected using a general information questionnaire,the Role Clarity Scale,the Professional Identity Assessment Scale,the Decent Work Perception Scale,and the Intent to Stay Scale.Correlation analysis,mediation analysis,and moderated mediation analysis were conducted using SPSS 27.0.Results:The median intent to stay score among medical nursing assistants was 13.00(12.00,16.00).Age,education level,daily working hours,and employment relationship were significant factors influencing intent to stay(P<0.05).Role clarity,professional identity,and decent work perception were all positively correlated with intent to stay(P<0.01).Professional identity partially mediated the relationship between role clarity and intent to stay,with an indirect effect value of 0.084(accounting for 39.25%of the total effect).Decent work perception negatively moderated the path from role clarity to professional identity(interaction effect=-0.047,P<0.001),such that the marginal contribution of role clarity to professional identity was stronger under low decent work perception.Conclusions:Role clarity not only directly and positively affects medical nursing assistants' intent to stay but also indirectly enhances their intent to stay through professional identity.Decent work perception serves as a moderator:under low decent work conditions,the protective effect of role clarity on professional identity becomes more pronounced.
