Absrtact This paper summarizes Professor DING Yuanqing's experience in treating migraine from the perspective of Jueyin.Professor DING posits that wind is the primary pathogenic factor in migraine,with the liver serving as the main diseased organ.The core pathogenesis involves Jueyin wind-fire ascending to disturb the clear orifices,leading to collateral obstruction and pain due to a lack of patency.Clinical manifestations at all stages are closely related to Jueyin.In clinical practice,it is essential to investigate internal causes and examine the patient's constitution.During the acute attack phase,the priority is to identify Jueyin wind-fire and differentiate between latent pathogens(such as phlegm and blood stasis)and the interplay of excess pathogens versus deficiency.Professor DING emphasizes a combined preventive and therapeutic approach.For acute attacks,the treatment focuses on calming the liver,regulating qi,dispelling wind,and unblocking collaterals.For chronic,intractable cases,emphasis is placed on dredging collaterals and protecting the brain.Treatment must be tailored to the individual,season,and location(the"three factors"),incorporating strategies such as nourishing yin,replenishing blood,softening the liver,resolving phlegm,activating blood,unblocking collaterals,and regulating the spirit.Based on pattern differentiation,modified classical formulas are pre-scribed,including Modified Baitouweng Decoction(白头翁汤加味),Xiaoxianxiong Decoction combined with Kulong Decoction(小陷胸汤合苦龙汤),Modified Gegen Qinlian Decoction(葛根芩连汤加味),Xuanfuhua Decoction combined with Dachuanxiong Pill(旋覆花汤合大川芎丸),and Xuanshen Tianma Decoction(玄参天麻汤).One test case is attached.
