转移性腺样囊性癌15例临床病理分析OA
Clinicopathological analysis of 15 cases of metastatic adenoid cystic carcinoma
目的 探讨转移性腺样囊性癌(adenoid cystic carcinoma,ACC)的临床病理特征、诊断及鉴别诊断.方法 回顾性分析15例转移性ACC的临床资料、组织学形态、免疫组化及MYB/NFIB 基因融合情况.结果 15例转移性ACC患者中男性7例,女性8例,年龄27~71岁,平均年龄50.6岁,转移部位包括肺、脑、肝、骨及其他器官.影像学提示转移部位单发或多发结节影.常规病理学检查显示:ACC由上皮细胞及肌上皮细胞形成管状、筛状和实性形态.免疫表型:腺上皮表达CK7、CK19、CD117,肌上皮表达SMA、S-100、p63及MYB,Ki67增殖指数中位数为25%.FISH检测显示:7例有MYB/NFIB基因融合.结论 腺样囊性癌可发生复发、转移,肺为最常见转移部位,亦可转移至骨、肝、脑等器官,临床易误、漏诊.临床病史、组织病理学、免疫组化及FISH检测联合应用可以提供重要线索,提高ACC的病理诊断准确性.
Objective To investigate the clinicopathological characteristics,diagnosis,and differential diagno-sis of metastatic adenoid cystic carcinoma(ACC).Methods The clinical data,histological features,immunohisto-chemical profiles,and MYB/NFIB gene fusion status of 15 metastatic ACC cases were retrospectively analyzed.Re-sults Among the 15 patients with metastatic ACC,7 were males and 8 were females,ranging in age from 27-71 years(mean:50.6 years).The metastatic sites included the lung,brain,liver,bone,and other organs.Imaging revealed single or multiple nodular shadows at the metastatic sites.Histologically,ACC was characterized by tubular,cribri-form,and solid patterns composed of ductal and myoepithelial cells.Immunohistochemically,the ductal cells were positive for CK7,CK19,and CD117,while the myoepithelial cells were positive for SMA,S-100,p63,and MYB.The median Ki67 proliferation index was 25%.FISH analysis showed MYB/NFIB gene fusion in 7 cases.Conclusion ACC has a high risk of distant metastasis,with the lung being the most common site,although metastases to the bone,liver,and brain may also occur.Due to its nonspecific presentation,metastatic ACC is prone to misdiagnosis.A com-bination of clinical history,histopathological evaluation,immunohistochemistry,and FISH testing can provide crucial diagnostic clues and improve diagnostic accuracy.
