透脓散联合四妙勇安汤加减在肛周脓肿术后的应用研究OA
Application of Modified Tounong Powder(透脓散)Combined with Modified Simiao Yong'an Decoction(四妙勇安汤)in Postoperative Treatment of Perianal Abscess
目的 研究透脓散联合四妙勇安汤加减在肛周脓肿术后的应用效果.方法 选取2023年11月—2024年4月河北省中医院肛肠中心收治的130例肛周脓肿患者作为研究对象,按随机数表法分为两组各65例,对照组给予静脉滴注头孢呋辛纳、氯化钠,并给予高锰酸钾溶液坐浴治疗,研究组在对照组的基础上给予透脓散联合四妙勇安汤加减治疗.两组均治疗3周,比较两组患者的疗效,中医证候积分,血管内皮生长因子(vascular endothelial growth factor,VEGF)、C反应蛋白(C-reactive protein,CRP)水平,治疗7、14、21 d,比较两组患者的创面愈合情况(创面分泌物、创面疼痛、创面肉芽评分).结果 研究组总有效率为95.38％(62/65),高于对照组[84.62％(55/65),P＜0.05];治疗7、14、21 d,研究组的创面分泌物、创面疼痛评分低于对照组,创面肉芽评分高于对照组(P＜0.05);研究组中医证候积分低于对照组(P＜0.05);研究组的VEGF水平高于对照组,CRP水平低于对照组(P＜0.05).结论 透脓散联合四妙勇安汤加减在肛周脓肿术后的应用效果显著,能够有效降低炎症反应,改善中医临床症状,减轻患者疼痛,促进创面愈合.
Objective To study the application effect of modified combination of Tounong Powder(透脓散)and Simiao Yong'an Decoction(四妙勇安汤)in the postoperative treatment of perianal abscess.Methods A total of 130 patients with perianal ab-scess in Colorectal Center of Hebei Hospital of Traditional Chinese Medicine from November 2023 to April 2024 were collected as the research subjects.They were randomly divided into two group according to a random number table,with 65 patients in each group.The control group received intravenous infusion of cefuroxime sodium and sodium chloride as well as potassium permanga-nate solution sitz bath treatment,and the study group received modified treatment with Tounong Powder and Simiao Yong'an De-coction on the basis of the control group.Both groups were treated for 3 weeks.The therapeutic effects,TCM syndrome scores,the levels of vascular endothelial growth factor(VEGF)and C-reactive protein(CRP)between two groups after treatment for 7,14 and 21 d were compared,the wound healing status of two groups(wound secretions,wound pain,wound granulation tissue score)were compared.Results The total effective rate of the study group was 95.38％(62/65),which was higher than that of the control group[84.62％(55/65),P＜0.05].After 7,14 and 21 d of treatment,the wound secretion and wound pain scores of the study group were lower than those of the control group,and the wound granulation score was higher than that of the control group(P＜0.05).The score of traditional Chinese medicine syndrome in the study group was lower than that in the control group(P＜0.05).The VEGF level in the study group was higher than that in the control group,and the CRP level was lower than that in the control group(P＜0.05).Conclusion The application effect of the combination of Tounong Powder and Simiao Yong'an Decoction after perianal abscess surgery is significant,which can effectively reduce the inflammatory reactions,improve the traditional Chi-nese medicine clinical symptoms,alleviate the pain and promote the wound healing.
