目的 探讨结直肠癌中HER2免疫组化表达及其与临床病理特征的关系.方法 收集结直肠癌患者的临床病理资料.采用免疫组化法检测HER2、MMR和BRAF V600E蛋白表达,FISH法检测HER2基因扩增,PCR法检测KRAS、NRAS、BRAF、PIK3CA基因热点突变,荧光PCR-毛细管电泳法检测微卫星不稳定性(microsatellite instability,MSI).采用χ2检验或Fisher精确检验评估HER2表达与临床病理特征的关系,并对有序变量采用线性趋势检验.结果 共收集1 971例结直肠癌患者,免疫组化HER2表达分布:0占58.0%,1+占34.9%,2+占5.6%,3+占1.5%.HER2阳性率为1.6%.HER2 3+表达与M1期(远处转移)显著相关(P=0.036).错配修复功能完整(proficient mismatch repair,pMMR)患者中HER2 3+占比(1.6%)高于错配修复功能缺陷(deficient mis-match repair,dMMR)(0),两者呈正相关(P<0.001).HER2不同表达与其他临床病理因素、BRAF V600E蛋白表达、KRAS/NRAS/BRAF基因突变及MSI状态无相关性(P>0.05).结论 HER2过表达(3+)是结直肠癌中一个与远处转移风险增加相关的特征,且其表达与左半结肠/直肠部位及pMMR状态相关.HER2表达模式与其他常规临床病理参数及RAS/BRAF等关键突变无关.

Objective To study the expression of HER2 in colorectal cancer(CRC)and its relationship with clinicopathological factors.Methods Clinicopathological data of colorectal cancer patients were collected.The ex-pressions of HER2,mismatch repair(MMR)proteins,and BRAF V600E protein were detected by immunohisto-chemital(IHC).HER2 gene amplification was detected by fluorescence in situ hybridization(FISH).Hotspot muta-tions of KRAS,NRAS,BRAF,and PIK3CA genes were detected by polymerase chain reaction(PCR),and microsat-ellite instability(MSI)was detected by fluorescence PCR-capillary electrophoresis.Chi-square(χ2)test or Fisher's exact test was used to evaluate the relationship between HER2 expression and clinicopathological factors,and the Mantel-Haenszel test was applied for ordinal variables.Results A total of 1 971 CRC cases were included in this study.The distribution of HER2 expression was as follows:0(58.0%),1+(34.9%),2+(5.6%),and 3+(1.5%).The overall positive rate of HER2 was 1.6%.HER2 3+was significantly associated with stage M1(distant metastasis)(P=0.036).The proportion of HER2 3+in patients with proficient mismatch repair(pMMR)(1.6%)was higher than that in patients with deficient mismatch repair(dMMR)(0),and the association was significant(P<0.001).There was no significant correlation between different HER2 expression levels and other clinicopathological factors,BRAF V600E protein expression,KRAS/NRAS/BRAF gene mutations,and MSI status(all P>0.05).Con-clusion HER2 overexpression(IHC 3+)is a feature associated with an increased risk of distant metastasis in CRC,and its expression correlates with left-sided/rectal tumor location and pMMR status.The HER2 expression pattern shows no significant correlation with other routine clinicopathological parameters or key mutations such as RAS/BRAF.
