目的:比较 Bio-DBM 骨塞与 Bio-Oss 骨粉进行牙槽嵴保存术(alveolar ridge preservation,ARP)与单纯拔牙后常规处理在维持牙槽嵴三维形态与骨再生的效果差异.方法:本研究为前瞻性临床试验,选取 2023 年 1 月~2024 年 12 月就诊于长沙市口腔医院修复科,行后牙拔牙后并考虑种植修复的患者 43 例(45 个种植位点),随机分为 3 组,实验组(同种异体骨组)使用 Bio-DBM 骨塞行牙槽嵴保存,对照组(异种骨)使用 Bio-Oss 骨粉行牙槽嵴保存,空白组:拔牙后常规处理(不进行 ARP).统计 3 组患者疼痛度、满意度、垂直向与水平向骨量变化,骨吸收量.结果:术后6 个月水平向骨量(距离牙槽嵴顶2 mm/4 mm/6 mm)实验组与对照组无显著差异(P>0.05),但均显著优于空白组(P<0.05);术后 6 个月垂直向骨量,实验组与对照组无显著差异(P>0.05),但均显著高于空白组(P<0.05);骨吸收量实验组与对照组无显著差异(P>0.05),但均显著低于空白组(P<0.05).结论:Bio-DBM 骨塞与Bio-Oss 骨粉在拔牙位点保存中均能有效维持牙槽嵴三维形态并促进高质量新骨形成,其疗效显著优于单纯拔牙处理,Bio-DBM 骨塞可作为 Bio-Oss 骨粉的可行替代方案,为种植修复提供可靠的骨基础.

Objective:To compare the efficacy of Bio-DBM bone plugs versus Bio-Oss bone granule for alveolar ridge preservation(ARP)against non-preservation in maintaining 3D ridge morphology and bone regeneration post-extraction.Methods:In this prospective trial,43 patients(45 sites)requiring posterior tooth extraction and implant restoration(2023~2024)were randomized into:experimental group(Bio-DBM bone plugs),control group(Bio-Oss bone granules),and blank group(extraction alone).Pain,satisfaction,vertical/horizontal bone dimensional changes,and bone resorption were assessed.Results:At 6 months postoperatively,horizontal bone mass(measured at 2 mm,4 mm,6 mm from the alveolar ridge crest)showed no significant difference between the experimental and control groups(P>0.05),but both were significantly superior to the blank group(P<0.05).Vertical bone mass at 6 months also showed no significant difference between the experimental and control groups(P>0.05),while both were significantly higher than the blank group(P<0.05).Bone resorption was simi-lar between the experimental and control groups(P>0.05),but both were significantly lower than that in the blank group(P<0.05).Conclusion:Both Bio-DBM plugs and Bio-Oss effectively preserve alveolar ridge dimensions and enhance bone regen-eration,outperforming extraction alone.Bio-DBM plugs offer a clinically equivalent alternative to Bio-Oss for implant site preparation.
