特发性震颤的病理生理机制及外科干预研究进展OA
Advances in pathophysiological mechanisms and surgical interventions for essential tremor
特发性震颤(ET)是一种以常染色体显性遗传模式为主的运动障碍性疾病,2018 年国际运动障碍学会共识将其明确定义为双侧上肢姿势性和/或动作性震颤,且症状持续≥3 年,可伴头部、声音或下肢震颤.其病理生理机制聚焦于小脑-丘脑-皮层环路功能异常,但具体分子通路尚未完全阐明,此病常与帕金森病震颤、功能性震颤、代谢性震颤和药物性震颤相鉴别.ET 病程呈隐匿性进展,早期轻度震颤可无需干预,而中重度患者需药物或外科治疗.本文系统综述 ET 的神经振荡机制、遗传与环境交互作用,以及外科干预新技术的循证进展,旨在为临床实践与研究方向提供综合性参考.
Essential tremor(ET)is a movement disorder with a predominantly autosomal dominant pattern of inheritance,which is clearly defined by the 2018 International Movement Disorder Society Consensus as a"bilateral postural and/or action tremor of the upper extremities"with symptoms persisting for≥3 years,which may be accompanied by head,voice or lower limb tremor.The pathophysiologic mechanism focuses on abnormal function of the cerebello-thalamo-cortical loop,but the specific molecular pathways have not been fully elucidated.It is often differentiated from Parkinson's disease tremor,functional tremor,metabolic tremor,and pharmacological tremor.The course of ET is insidious,with mild tremor requiring no intervention in the early stages of the disease,and moderate-to-severe patients requiring pharmacological or surgical treatment.This article provides a systematic review of the neural oscillatory mechanisms of ET,genetic and environmental interactions,and evidence-based advances in new techniques for surgical intervention,with the aim of providing a comprehensive reference for clinical practice and research orientation.
