目的 探讨内镜经枕天幕-松果体隐窝-上髓帆入路清除全脑室血肿的可行性及临床效果.方法 回顾性分析苏州大学附属苏州九院神经外科2024 年11 月收治的1 例丘脑出血破入脑室的病例,采用内镜下经枕天幕-松果体隐窝-上髓帆入路清除丘脑及全脑室血肿.结果 内镜下经枕天幕-松果体隐窝-上髓帆入路可直视下清除丘脑及全脑室血肿,早期打通脑脊液循环通路,可早期改善患者意识障碍.患者未行脑室外引流术,术后未并发再出血、脑积水及颅内感染,取得了满意的血肿清除效率及临床效果.结论 内镜经枕天幕-松果体隐窝-上髓帆入路可避免放置脑室外引流及脑室内注射尿激酶,具有潜在应用优势,可进一步应用、推广.

Objective To explore the feasibility and clinical efficacy of endoscopic hematoma evacuation via transoccipital tentorium-pineal recess-superior medullary velum approach for whole ventricular hematoma.Methods A retrospective analysis was conducted on one case of thalamic hemorrhage rupturing into the ventricles admitted to the Department of Neurosurgery,Suzhou Ninth Hospital Affiliated to Soochow University in November 2024.Hematomas in the thalamus and entire ventricles were evacuated via the endoscopic transoccipital tentorium-pineal recess-superior medullary velum approach.Results The endoscopic transoccipital tentorium-pineal recess-superior medullary velum approach allowed direct evacuation of hematomas in the thalamus and whole ventricular system.Early establishment of cerebrospinal fluid circulation improved patients'conscious disturbance rapidly.No external ventricular drainage was conducted.No postoperative complications such as rebleeding,hydrocephalus and intracranial infection were observed.Favorable hematoma clearance and clinical efficacy were obtained.Conclusions The endoscopic transoccipital tentorium-pineal recess-superior medullary velum approach avoids external ventricular drainage and intraventricular urokinase injection.It possesses potential clinical advantages and deserves further application and promotion.
