蛛网膜下腔出血并发癫痫的因素探讨及风险预测Nomogram模型构建OA

Objective To analyze the influencing factors of subarachnoid hemorrhage(SAH)complicated with epilepsy,and construct a risk prediction Nomogram model.Methods The clinical data of 319 SAH patients admitted to Langfang People's Hospital from January 2022 to January 2023 were retrospectively analyzed,and epilepsy was diagnosed in accordance with the relevant clinical criteria.The predictive variables were screened by stepwise forward method and stepwise backward regression method,and the Nomogram model was constructed and verified by R3.4.3 software package.Results The incidence of epilepsy in 319 SAH patients was 19.44%(62/319).Hypertension,hydrocephalus,history of cerebral infarction,Fisher score and Hunt-Hess classification were selected as the influencing factors of SAH complicated with epilepsy,so the Nomogram model was constructed,and the ROC curve analysis showed that the areas under the curve of the model for predicting epilepsy in the modeling group and the verification group were 0.960(0.926-0.982)and 0.859(0.773-0.922),respectively,which had good calibration,and it could achieve clinical net benefits within most threshold probabilities.Conclusions The risk prediction Nomogram model constructed in this study can well predict the occurrence of epilepsy after SAH and guide prophylactic measurements.
