约50%的结直肠癌(CRC)患者可发生肝转移,昼夜节律紊乱是其重要危险因素,但肠道微生态与免疫微环境的协同调控机制尚未阐明.近年来,"肠道菌群-免疫轴"为解析该过程提供了全新理论框架.节律紊乱可升高Enterobacteriaceae丰度、降低Roseburia及Akkermansia丰度,并伴随脱氧胆酸(DCA)与牛磺胆酸(TCA)异常蓄积.DCA通过质膜钙离子转运ATP酶-活化T细胞核因子2(PMCA-NFAT2)通路直接抑制CD8+T细胞杀伤功能;TCA则分别驱动中性粒细胞与髓源性抑制细胞(MDSCs)向肝脏募集,二者协同构建免疫抑制性转移前微环境.本文系统梳理了昼夜节律紊乱经肠道菌群-免疫轴驱动结直肠癌肝转移(CRLM)的核心调控通路及研究进展.基于该轴的时序治疗、益生菌干预及粪菌移植(FMT)等策略有望成为CRLM防治的新方向,而人鼠菌群差异与肝转移灶免疫异质性仍是临床转化的关键瓶颈.

Approximately 50%of patients with colorectal cancer(CRC)develop liver metastases,with circadian rhythm disruption serving as a significant risk factor;however,the synergistic regulatory mechanisms between the gut microbiota and immune microenvironment remain incompletely elucidated.In recent years,the"gut microbiota-immune axis"has provided a novel theoretical framework for dissecting this process.Circadian rhythm disruption increases the abundance of Enterobacteriaceae while decreasing that of Roseburia and Akkermansia,accompanied by aberrant accumulation of deoxycholic acid(DCA)and taurocholic acid(TCA).DC A directly suppresses the cytotoxic function of CD8+T cells via the plasma membrane Ca2+-transporting ATPase-nuclear factor of activated T cells 2(PMCA-NFAT2)pathway,whereas TCA drives the hepatic recruitment of neutrophils and myeloid-derived suppressor cells(MDSCs),which collaboratively establish an immunosuppressive pre-metastatic niche.This paper systematically reviewed the core regulatory pathways and research progress of circadian rhythm disorders driving colorec-tal cancer liver metastasis(CRLM)via the gut microbiota-immune axis.Chronotherapy,probiotic inter-ventions,and fecal microbiota transplantation(FMT)based on this axis are highlighted as promising new directions for CRLM prevention and treatment.However,discrepancies between human and murine microbiota,along with the immune heterogeneity of liver metastases,remains key bottlenecks for clinical translation.
