1例整合性多学科协作模式下阴茎完全离断伤的显微外科再植病例报告OA

Objective To explore the application value of the integrated multidisciplinary col-laboration(MDC)model in the treatment of patients with complete penile amputation injuries.Meth-ods A retrospective analysis was conducted on the clinical data of a patient with complete penile am-putation due to a sawing injury in the Tangdu Hospital of Air Force Medical University in April 2025.The integrated MDC model,led by the Departments of Urology and Plastic Surgery,was employed,debridement,urethral and corporal cavernosum anastomosis,vascular and nerve anastomosis,and scrotal aesthetic repair under microsurgical techniques were conducted.Postoperatively,comprehen-sive treatment including anti-infection,anti-vasospasm,anticoagulation,and fluid resuscitation was administered.Results Immediate restoration of blood supply to the replanted penis was achieved postoperatively.Early dressing changes revealed swelling of the replanted penis,which subsequently improved.The urinary catheter was removed on postoperative day 23,with unobstructed urination and primary wound healing at the surgical site upon discharge.At the 4-month follow-up,the patient re-ported satisfactory sexual function,smooth urination,and the absence of urethral stricture or urinary fistula.Sensory and erectile function of the distal penis recovered,with an International Index of Erectile Function Questionnaire-5(IIEF-5)score of 18.Conclusion This case demonstrates that the integrated MDC model,constructed around patient needs and emphasizing complementary advantages of specialized techniques,facilitates high-quality rehabilitation in cases of complete penile amputa-tion.This model systematically combines functional reconstruction goals with morphological repair techniques,providing valuable experiencefor managing other complex compound organ injuries.
