热敏灸机器人变频循经往返灸治疗非特异性腰痛随机对照研究OA
Randomized Controlled Study on the Treatment of Non-specific Low Back Pain Using Frequency-Conversion Shuttle Moxibustion Along Meridians with a Heat-sensitive Moxibustion Robot
目的:观察热敏灸机器人变频循经往返灸对非特异性腰痛(NSLBP)的临床治疗效果,比较不同参数循经往返灸得气效应的特异性,为临床个体化施灸手法的选择提供科学依据.方法:收集 2024 年 10 月—2025 年 2 月在广东省中西医结合医院门诊、住院治疗的 NSLBP 患者 108 例,随机分为变频热敏循经往返灸组(变频组)、非变频热敏循经往返灸 1 组(非变频 1 组)、非变频热敏循经往返灸 2 组(非变频 2 组),各 36 例.3 组分别进行不同参数的循经往返灸治疗,并对 3 组患者治疗过程中的艾灸得气与感传情况,治疗前后的视觉模拟量表(VAS)、现有疼痛强度(PPI)、Roland-Morris 功能障碍(RMDQ)评分及临床疗效进行比较.结果:比较 3 组得气潜伏期、得气累积时间、得气总积分、艾灸感传视觉模拟评分,变频组与非变频 1 组、非变频 2 组比较均有显著性差异(P<0.05),非变频 1 组与非变频 2 组比较无显著性差异(P>0.05).治疗后,3 组 VAS、PPI、RMDQ 评分与治疗前比较均有显著性差异(P<0.01);变频组的 VAS、PPI、RMDQ 评分与非变频 1 组、非变频 2 组比较均有显著性差异(P<0.01),非变频 1 组与非变频 2 组比较无显著性差异(P>0.05).治疗后,变频组与非变频 1 组、非变频 2 组的临床愈显率比较均有显著性差异(P<0.05),非变频 1 组与非变频 2 组比较无显著性差异(P>0.05);3 组总有效率比较无显著性差异(P>0.05).结论:热敏灸机器人施行变频循经往返灸手法在激发NSLBP 患者艾灸得气、改善临床症状上优于非变频手法.
Objective:To evaluate the clinical efficacy of a heat-sensitive moxibustion robot that performs frequency-conversion Shuttle Moxibustion along Meridians in patients with non-specific low back pain(NSLBP),to characterise the Deqi specificity of distinct round-trip parameters,and to provide an evidence base for individualised moxibustion protocols.Methods:108 participants enrolled with NSLBP who received outpatient and inpatient treatment at Guangdong Provincial Hospital of Integrated Traditional Chinese and Western Medicine from October 2024 to February 2025,were randomly allocated to three groups(n=36 each):(1)frequency-conversion heat-sensitive shuttle moxibustion along meridians group(FC),(2)non-frequency-conversion heat-sensitive shuttle moxibustion along meridians group 1(NFC-1),and(3)non-frequencyconversion heat-sensitive shuttle moxibustion along meridians group 2(NFC-2).All interventions were delivered robotically with parameter sets unique to each group.Deqi and sensation transmission for moxa-induced sensation were recorded during treatment.Visual Analogue Scale(VAS),Present Pain Intensity(PPI),and Roland-Morris Disability Questionnaire(RMDQ)scores were obtained before and after treatment;clinical efficacy was compared across groups.Results:Inter-group comparisons revealed significant heterogeneity in Deqi latency,cumulative Deqi duration,total Deqi score,and moxa-sensation(P<0.05).Post-hoc analyses indicated that the FC group cohort outperformed both in NFC-1 group and NFC-2group(P<0.05),whereas the two non-frequency-conversion subgroups exhibited negligible mutual divergence(P>0.05).After therapy,VAS,PPI,and RMDQ values declined markedly within every group(P<0.01);the FC group achieved significantly greater reductions than the NFC-1 and NFC-2 groups(P<0.01),while no significant difference was observed between the NFC-1 and NFC-2 groups(P>0.05).After therapy,in terms of clinical remission rate,again favouring FC group over either fixed-speed protocol(P<0.05),while NFC-1 group and NFC-2 group remained statistically indistinguishable(P>0.05).Total effective rates did not differ among the three cohorts(P>0.05).Conclusion:Frequency-conversion shuttle moxibustion along meridians implemented by a heat-sensitive robotic system elicits a more robust Deqi response and affords superior symptomatic relief in NSLBP compared with non-frequency-conversion techniques,supporting its preferential adoption for individualised clinical application.
