温肾健脾方联合蒸脐疗法对IBS-D患者炎性因子、肠道菌群的影响OA
Effect of Wenshen Jianpi Formula Combined with Umbilical Steaming Therapy on Inflammatory Factors and Gut Microbiota in Patients with Diarrhea-predominant Irritable Bowel Syndrome
目的:观察采用温肾健脾方联合蒸脐疗法治疗腹泻型肠易激综合征(IBS-D)患者的临床疗效及对炎性因子、肠道菌群的影响.方法:选取 2022 年 1 月—2023 年 12 月江西中医药大学附属医院门诊及住院部收治的符合纳入标准的脾肾阳虚证 IBS-D 患者 80 例,随机分为 2 组,观察组 40 例,采用温肾健脾方联合蒸脐疗法治疗;对照组40 例,采用口服双歧杆菌三联活菌胶囊治疗,均治疗4周为1个疗程.治疗4周后,比较2组治疗前后中医证候总积分、血清炎性因子肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、肠道菌群、肠易激综合征生活质量量表(IBS-QOL)评分的变化,比较中医证候疗效.结果:治疗后,观察组总有效率为 95.00%,高于对照组的 67.50%(P<0.05);2 组中医证候总积分、IBS-QOL评分,血清IL-6、IL-8、TNF-α水平,肠杆菌、肠球菌数量均显著下降,且观察组均低于对照组(P<0.01);2 组血清 IL-10 水平及双歧杆菌、乳杆菌数量均升高,且观察组均高于对照组(P<0.01).结论:温肾健脾方联合蒸脐疗法治疗IBS-D疗效显著,可改善临床症状,其作用机制可能与调节血清炎性因子、肠道菌群有关.
Objective:To observe the clinical efficacy of Wenshen Jianpi Formula combined with umbilical steaming therapy in the treatment of patients with Diarrhea-predominant Irritable Bowel Syndrome(IBS-D)of spleen-kidney Yang deficiency syndrome,and investigate its effects on inflammatory factors and gut microbiota.Methods:A total of 80 IBS-D patients with spleen-kidney yang deficiency syndrome who met the inclusion criteria were enrolled in the outpatient and inpatient departments of The Affiliated Hospital of Jiangxi University of Chinese Medicine from January 2022 to December 2023.The patients were randomly divided into two groups,with 40 cases in the observation group and 40 cases in the control group.The observation group was treated with Wenshen Jianpi Formula combined with umbilical steaming therapy,while the control group was treated with oral Bifidobacterium Triple Viable Capsules.The treatment course for both groups was 4 weeks.After 4 weeks of treatment,the changes of total TCM syndrome score,serum inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10(IL-10)],gut microbiota,and Irritable Bowel Syndrome Quality of Life(IBS-QOL)scale score before and after treatment were compared between the two groups,and the curative effect on TCM syndromes was evaluated.Results:After treatment,the total effective rate of the observation group(95.00%)was significantly higher than that of the control group(67.50%)(P<0.05).The total TCM syndrome score,IBS-QOL score,serum levels of IL-6,IL-8 and TNF-α,as well as the counts of Enterobacter and Enterococcus in both groups were significantly decreased after treatment,and the above indicators in the observation group were lower than those in the control group(P<0.01).The serum level of IL-10 and the counts of Bifidobacterium and Lactobacillus in both groups were increased after treatment,and those in the observation group were significantly higher than the control group(P<0.01).Conclusion:Wenshen Jianpi Formula combined with umbilical steaming therapy has a significant curative effect in the treatment of IBS-D and can relieve the clinical symptoms of patients.Its mechanism of action may be related to the regulation of serum inflammatory factors and gut microbiota.
