目的 探讨血流向量成像技术(VFM)联合二维斑点追踪技术(2D-STI)评估冠心病患者增强型体外反搏(EECP)治疗后早期左心功能指标变化的价值.方法 将84例在郑州大学第二附属医院心内科治疗的左室射血分数正常的冠心病患者随机分为EECP组(n=42)和对照组(n=42).EECP组进行药物治疗联合EECP治疗,对照组仅进行药物治疗.入院时和治疗3.5、7周时行超声心动图检查,测量所有研究对象的常规心功能参数,并采用VFM及2D-STI分析图像,测得等容舒张期(IR)、快速充盈期(RF)、减慢充盈期(SF)及心房收缩期(AC)各舒张期左室内压力差(IVPD)、压力梯度(IVPG)以及左室整体纵向应变(GLS).分析2组各时点上述指标的差异及特征.结果 治疗3.5周时,EECP组和对照组GLS绝对值较入院时增大,IR、RF、AC的IVPD、IVPG较入院时增加,差异有统计学意义(P<0.05),Tei指数、二尖瓣环间隔处和左室侧壁处最大速度(e')、左房容积指数(LAVi)、舒张二尖瓣早期峰值流速(E)、E/e'、舒张晚期二尖瓣峰值流速(A)、E/A、左室射血分数与入院时比较,差异无统计学意义(P>0.05).治疗7周时,EECP组和对照组的GLS、IVPD、IVPG绝对值较治疗3.5周时进一步增大,Tei指数减小,e'增大,E/e'减小,差异均有统计学意义(P<0.05).治疗7周时,2组IVPD-IR、IVPD-RF、IVPD-SF、IVPD-AC、IVPG-IR、IVPG-RF、IVPG-SF、IVPG-AC、GLS、Tei 指数和 E/e'差异有统计学意义(P<0.05).RF、AC 的 IVPD、IVPG 与 e'、GLS呈正相关,与E/e'呈负相关,且相关性结果较其他时期更显著.结论 VFM联合2D-STI可有效评估CAD患者EECP治疗后早期左心功能改变.

Objective To investigate the value of vector flow mapping(VFM)combined with two-dimensional speckle tracking imaging(2D-STI)in evaluating early changes in left ventricular function indi-ces after enhanced external counterpulsation(EECP)therapy in patients with coronary artery disease(CAD).Methods A total of 84 CAD patients with normal left ventricular ejection fraction(LVEF)trea-ted in the Department of Cardiology,the Second Affiliated Hospital of Zhengzhou University,were randomly divided into EECP group(n=42)and control group(n=42).The EECP group received pharmacological therapy combined with EECP,while the control group received pharmacological therapy alone.Echocardiography was performed at admission and at 3.5 and 7 weeks of treatment to measure con-ventional cardiacfunction parameters in all subjects.VFM and 2D-STI techniques were used to ana-lyze the images,and the following parameters were measured:intraventricular pressure difference(IVPD)and pressure gradient(IVPG)during isovolumic relaxation(IR),rapid filling(RF),slow filling(SF),and atrial contraction(AC)phases,as well as global longitudinal strain(GLS)of the left ventricle.The differences and characteristics of these indices between the two groups at each time point were analyzed.Results At 3.5 weeks of treatment,the absolute values of GLS in both the EECP and control groups increased compared with those at admission,and the IVPD and IVPG during IR,RF and AC phases also increased,with statistically significant differences(P<0.05).No significant differences were observed in the Tei index,maximal velocity(e')at the mi-tral annulus(interventricular septum and left ventricular lateral wall),E/e',left atrial volume in-dex(LAVi),peak early diastolic mitral velocity(E),peak late diastolic mitral velocity(A),E/A,or LVEF compared with those at admission(P>0.05).At 7 weeks of treatment,the absolute val-ues of GLS,IVPD,and IVPG in both groups further increased compared with those at 3.5 weeks,while the Tei index decreased,e'increased,and E/e'decreased,with statistically significant differ-ences(P<0.05).At 7 weeks,significant differences were observed between the two groups in IVPD-IR,IVPD-RF,IVPD-SF,IVPD-AC,IVPG-IR,IVPG-RF,IVPG-SF,IVPG-AC,GLS,Tei index,and E/e'(P<0.05).The IVPD and IVPG during RF and AC phases were positively corre-lated with e'and GLS and negatively correlated with E/e',with more pronounced correlations ob-served compared with other periods.Conclusion VFM combined with 2D-STI can effectively evalu-ate early changes in left ventricular function after EECP therapy in patients with CAD.
