新生儿低血糖多种临床特征与影响因素相关性OA
Correlation between diverse clinical features and their influencing factors in neonatal hypoglycemia
目的 探讨新生儿低血糖(NH)多种临床特征的影响因素与预测价值.方法 纳入2021年8月至2023年8月山西省儿童医院新生儿内科病房收治的194例NH患儿作为对象进行回顾性分析.建立多元线性回归模型分析NH不同临床特征(首次出现时间、首发血糖值、低血糖持续时间、最低血糖值)与多种影响因素之间的联系.结果 母亲患有高血压、低钙血症者NH首次出现时间较晚(β值分别为1.153、1.882,P<0.05),母亲患有糖尿病者 NH首次出现时间较早(β=-0.859,P<0.05);新生儿患有动脉导管未闭(PDA)者 NH 首发血糖值较高(β=0.185,P<0.05),患儿开奶方式为配方奶喂养者、有眼底出血者NH首发血糖值较低(β值分别为-0.220、-0.619,P<0.05);患有低钾血症的 NH 持续时间更长(β=1.372,P<0.05),开奶方式为配方奶的新生儿相较于混合喂养的新生儿 NH持续时间更短(β=-0.788,P<0.05);巨大儿组新生儿最低血糖值较高(β=0.506,P<0.05),母亲患有子痫前期、新生儿呼吸衰竭、眼底出血、喂养方式为配方奶的患儿出生后最低血糖值较低(β值分别为-0.211、-0.317、-0.506、-0.288,P<0.05).结论 新生儿呼吸衰竭、孕母合并高血压、子痫前期、妊娠期糖尿病是新生儿出现更严重新生儿低血糖的危险因素.当母乳喂养不足时,采取混合喂养,有利于防治更严重新生儿低血糖,改善预后.
Objective To investigate influencing factors and predictive values of diverse clinical features of neonatal hypoglycemia(NH).Methods A total of 194 neonates with NH who admitted to Department of Internal Neonatology of Shanxi Children's Hospital from August 2021 to August 2023 were included in retrospective study.Multiple linear regression models were established to analyze relationships between different diverse clinical features of NH(first onset time of hypoglycemia,blood glucose value at first hypoglycemia onset,duration of hypoglycemia,the lowest value of blood glucose)and their influencing factors.Results The first NH onset times of the neonates of their mothers who suffered from hypertensive disorder of pregnancy(HDP)and hypocalcemia during pregnancy were later(β=1.153 and 1.882 respectively,both P<0.05),while that of the neonates of their mothers who suffered from gestational diabetes mellitus(GDM)was earlier(β=-0.859,P<0.05).The neonates with patent ductus arteriosus(PDA)had higher blood glucose value at first onset of NH(β=0.185,P<0.05),whereas those neonates who were fed initially with formula milk powder or the neonates had retinal hemorrhage had lower blood glucose values at first onset of NH(β=-0.220 and-0.619 respectively,both P<0.05).The duration of NH of these neonates with hypokalemia was longer(β=1.372,P<0.05),while the duration of NH of those neonates who were fed with formula milk powder was shorter compared to those neonates who received mixed feeding(β=-0.788,P<0.05).The lowest blood glucose value of the macrosomias was higher(β=0.506,P<0.05),whereas the lowest blood glucose values of those neonates whose mother suffered from preeclampsia,or the neonates had neonatal respiratory failure or retinal hemorrhage,and those neonates were fed with formula milk powder after birth were lower(β=-0.211,-0.317,-0.506 and-0.288 respectively,all P<0.05).Conclusion Neonate having respiratory failure,mother suffering from HDP,preeclampsia and GDM are all risk factors for occurrence of more severe NH.When mother's breast milk is insufficient,mixed feeding could be chosen,which is beneficial for preventing more severe NH and improving prognosis of the disease.
