正弦谐波加速试验在周围性眩晕临床分型中的应用OA
Application of sinusoidal harmonic acceleration test in clinical classification of peripheral vertigo
目的 探讨正弦谐波加速试验(sinusoidal harmonic acceleration test,SHAT)的频率特异性,及其在周围性眩晕临床分型中的应用价值.方法 选取120例周围性眩晕患者和40名健康对照.根据冷热试验结果将患者分为双侧对称组、双侧不对称组和双侧减退组,完成SHAT并记录各频率下的增益和相移.结果 各组在多个频率下增益和相移存在显著差异(P<0.05),以低频段最为显著;SHAT结果与CP值存在显著相关性,以0.01 Hz增益最为明显(r=-0.772,P<0.001);等价性分析显示,0.02、0.08、0.64 Hz三频率组合与全频率检查结果具有最强相关性(增益值r=0.994,相移值r=0.867,P均<0.001),组内相关系数分析显示两种方法具有极高的一致性(ICC=0.995,P<0.001),可作为快速筛查方案.结论 SHAT频率特异性分析可为周围性眩晕的临床分型提供客观、可靠的诊断依据.
OBJECTIVE To investigate the frequency specificity of sinusoidal harmonic acceleration test(SHAT)and its clinical value in the classification of peripheral vertigo.METHODS 120 patients with peripheral vertigo and 40 healthy controls were selected.Based on caloric test results,patients were divided into bilateral symmetrical group,bilateral asymmetrical group,and bilateral hypofunction group.SHAT was performed,and gain and phase values were recorded at different frequencies.RESULTS Significant differences in gain and phase were observed among groups at multiple frequencies(P<0.05),most notably in the low-frequency range.SHAT results showed significant correlation with CP values,with gain at 0.01 Hz demonstrating the strongest correlation(r=-0.772,P<0.001).Equivalence analysis revealed that the three-frequency combination 0.02,0.08,0.64 Hz showed the strongest correlation with full-frequency examination results(gain r=0.994,phase r=0.867,both P<0.001).Intraclass correlation coefficient analysis indicated high consistency between the two methods(ICC=0.995,P<0.001),suggesting its potential as a rapid screening protocol.CONCLUSION Frequency specificity analysis of SHAT can provide objective and reliable diagnostic criteria for clinical classification of peripheral vertigo.
