目的:研究芪蒡糖肾方对糖尿病肾病小鼠及高糖诱导的人肾皮质近曲小管上皮细胞HK2纤维化的保护作用.方 法:(1)体内实验:选取C57BL/6J雄性小鼠,采用高糖高脂饲料喂养+链脲霉素注射方法建立糖尿病肾病动物模型.造模成功后,将其分为正常组、模型组、达格列净组、芪蒡糖肾方低剂量组、芪蒡糖肾方中剂量组、芪蒡糖肾方高剂量组.连续给药 12周,测定各组 24 h尿蛋白(24 hpro)、血肌酐(serum creatinine,Scr)、空腹血糖(fasting blood glucose,FBG)及肾组织内亚铁离子(Fe2+)水平.采用Masson染色观察肾组织病理损伤程度.采用 Western blot检测 Nrf2/Gpx4信号途径及下游纤维化相关蛋白的表达水平.(2)体外实验:采用CCK-8检测细胞活力,筛选芪蒡糖肾方最佳干预浓度.采用Western blot检测Col3a1表达水平,确定造模条件.将HK2细胞分为正常组、高糖组、芪蒡糖肾方组、达格列净组.检测细胞内丙二醛(malondialdehyde,MDA)、谷胱甘肽(glutathione,GSH)、Fe2+、活性氧(reactive oxygen species,ROS)的浓度与Nrf2/Gpx4信号途径及下游纤维化相关蛋白的表达水平.透射电子显微镜观察HK2细胞超微结构变化.结 果:(1)体内实验:与模型组相比,芪蒡糖肾方干预后,24 hpro、FBG、Scr以及肾组织内Fe2+水平下降(P<0.01).除此之外,芪蒡糖肾方还能促进肾组织内Nrf2、Gpx4、E-Cadherin、Slc7a11等蛋白的表达并降低α-SMA、Col3a1及Vimentin的表达(P<0.05).(2)体外实验:与高糖组相比,芪蒡糖肾方能明显降低细胞内的MDA、Fe2+和ROS浓度,改善线粒体损伤,同时增加细胞内GSH的含量(P<0.01).除此之外,芪蒡糖肾方还能促进细胞内 Nrf2、Gpx4、Slc7a11和 E-Cadherin蛋白的表达,而抑制 α-SMA、Col3a1和 Vimentin蛋白的表达(P<0.01).结论:芪蒡糖肾方有助于改善糖尿病肾病肾纤维化,这可能与Nrf2/Gpx4信号传导途径的活化有关.

Objective:To investigate the protective effect of Qibang Tangshen Formula(QBTSF)on diabetic nephropathy mice and high glucose-induced fibrosis of human renal cortical proximal tubular epithelial cells HK2.Methods:(1)In vivo experi-ments:C57BL/6 J male mice were selected to establish an animal model of diabetic nephropathy by using high-sugar and high-fat feed+streptozocin injection,and after successful modeling,the mice were divided into a normal group,a model group,a dapa-gliflozin group,a QBTSF low-dose group,a QBTSF middle-dose group,and a QBTSF high-dose group.The drug was adminis-tered continuously for 12 weeks,and the levels of 24 h urinary protein(24 hpro),serum creatinine(Scr),fasting blood glucose(FBG),and ferrous ion(Fe2+)in renal tissues were measured in each group.Masson staining was used to observe the degree of renal histopathological damage.Western blot was used to detect the expression levels of Nrf2/Gpx4 signaling pathway and down-stream fibrosis-related proteins.(2)In vitro experiments:CCK-8 was used to detect cell viability and screen the optimal interven-tion concentration of QBTSF.Western blot was used to detect the expression level of Col3a1 and determine the modeling condi-tions.The HK2 cells were divided into a normal group,a high glucose group,a QBTSF group,and a dapagliflozin group.The in-tracellular concentrations of malondialdehyde(MDA),glutathione(GSH),Fe2+,and reactive oxygen species(ROS)were mea-sured,and the expression levels of Nrf2/Gpx4 signaling pathway and fibrosis-related proteins were analyzed.Transmission elec-tron microscopy was used to observe the ultrastructural changes of HK2 cells.Results:(1)In vivo experiment:Compared to the model group,after the intervention of QBTSF,24 hpro,FBG,Scr,and Fe2+levels in renal tissues were decreased(P<0.01).In addition,QBTSF could promote the expression of Nrf2,Gpx4,E-Cadherin,and Slc7a11 proteins,as well as reduce the ex-pression of α-SMA、Col3a1,and Vimentin proteins in renal tissues(P<0.05).(2)In vitro experiments:Compared to the high glucose group,QBTSF could significantly reduce intracellular MDA,Fe2+,and ROS concentrations and improve mitochondrial damage while increasing intracellular GSH content(P<0.01).In addition,QBTSF promoted the expression of intracellular Nrf2,Gpx4,Slc7a11,and E-Cadherin proteins,while inhibiting the expression of α-SMA,Col3a1,and Vimentin proteins(P<0.01).Conclusion:QBTSF helps to improve renal fibrosis in diabetic nephropathy,which may be associated with the activation of Nrf2/Gpx4 signaling pathway.
