基于异常机械应力探讨中医药调控髓核细胞铁死亡防治椎间盘退变的研究进展OA
Research Progress on the Regulation of Ferroptosis in Nucleus Pulposus Cells by Traditional Chinese Medicine for the Prevention and Treatment of Intervertebral Disc Degeneration Based on Abnormal Mechanical Stress
髓核细胞(NPCs)数量的进行性减少是驱动椎间盘退行性疾病(IVDD)发生、发展的核心病理环节,异常机械应力通过机械敏感通道Piezo1介导的"双路径并发"(铁代谢异常与线粒体损伤)触发NPCs铁死亡与IVDD的加速进展密切相关.中医药调控NPCs铁死亡防治IVDD的机制研究近年来成为前沿热点之一,中药复方如六味地黄丸、附子汤、独活寄生汤、身痛逐瘀汤等,中药单体如扁柏酚、橙皮苷、没食子酸、洋蓟素、淫羊藿苷等,以及针灸、电针等中医外治法,可能通过干预Piezo1通道介导的力学-生物学转化,上调膜铁转运蛋白(FPN)、金属调节转录因子1(MTF1)等蛋白表达以恢复细胞铁稳态,抑制活性氧(ROS)产生以修复线粒体功能,并激活核因子E2相关因子2(Nrf2)、谷胱甘肽过氧化物酶4(GPX4)、沉默信息调控因子1(SIRT1)等核心抗氧化通路,从而有效阻断脂质过氧化链式反应,抑制NPCs铁死亡,发挥防治IVDD的作用.建议今后突破"单一靶点-线性通路"的还原论思维,结合中医"肾精亏虚证""寒湿痹阻证"等特定证型的病理特点,深入开展"病-证-靶点"结合的模型构建与系统性机制研究,为阐明中医药防治IVDD的机制及创新药物研发提供新路径.
The progressive reduction in the number of nucleus pulposus cells(NPCs)is a central pathological event in the onset and progression of intervertebral disc degeneration(IVDD).Aberrant mechanical stress may accelerate the progression of IVDD by triggering ferroptosis in NPCs via the mechanosensitive ion channel Piezo1,thereby inducing concurrent dysregulation of iron metab-olism and mitochondrial damage.Therefore,elucidating the mechanisms by which Traditional Chinese Medicine(TCM)modulates NPC ferroptosis has become an emerging focus in IVDD research.Emerging evidence suggests that classical TCM formulas,such as Liuwei Dihuang Pill,Fuzi Decoction,Duhuo Jisheng Decoction,and Shentong Zhuyu Decoction,bioactive compounds,such as hinok-itiol,hesperidin,gallic acid,cynarin,and icariin,and external therapies,like acupuncture and electroacupuncture,may modulate Piezo1-mediated mechanical-biological transduction.Mechanistically,these interventions may the expression of upregulate ferropor-tin(FPN)and metal-regulatory transcription factor 1(MTF1)to restore cellular iron homeostasis,suppress the production of reactive oxygen species(ROS),and help repair mitochondrial function.By activating key antioxidant pathways involving nuclear factor ery-throid 2-related factor 2(Nrf2),glutathione peroxidase 4(GPX4),and sirtuin 1(SIRT1),TCM interventions may block lipid peroxida-tion chain reactions,inhibit NPC ferroptosis,and contribute to the prevention and treatment of IVDD.Future research should move beyond the reductionist paradigm of"single target and linear pathway".Future studies should integrate the pathological characteristics of specific TCM patterns,such as Kidney essence deficiency and cold-dampness impediment,and adopt a holistic"disease-pattern-tar-get"approach for model construction and systematic mechanistic investigation.Such systems-level methodologies may provide new insights into the mechanisms by which TCM prevents and treats IVDD and may offer new strategies for innovative drug development.
