基于术前CT三维重建的上颌窦良性病变定位对经鼻内镜不同入路选择的指导价值研究OA
Guiding Value of Preoperative CT Three-dimensional Reconstruction-based Localization of Benign Maxillary Sinus Lesions for Selecting Transnasal Endoscopic Surgical Approaches
目的:定量评估基于术前电子计算机断层扫描(CT)三维重建的上颌窦良性病变定位,相较于传统二维CT联合鼻内镜的经验决策,在中鼻道联合下鼻道开窗入路与泪前隐窝入路选择中的指导价值,为临床精准手术提供依据.方法:选取本院2021年1月至2024年7月收治的116例上颌窦良性病变患者,根据随机数字表法分为两组(各58例).三维重建组(n=58)基于术前CT三维重建定位确定手术入路,对照组(n=58)基于常规二维CT轴位/冠状位影像+鼻内镜检查结果选择入路.比较两组患者的术中调整入路决策的发生率、病变完整切除率、手术指标(术中出血量、手术时间、住院时间)、疼痛视觉模拟评分(visual analogue scale,VAS)、并发症发生率、临床疗效及1年复发率.结果:三维重建组并发症发生率、复发率低于对照组(P<0.05),病变完整切除率高于对照组(P<0.05),术中出血量少于对照组(P<0.05);两组疼痛VAS评分:术后24h>6h>48 h>3个月(P<0.05),三维重建组术后6h、24h、48h VAS评分低于对照组(P<0.05),术后3个月组间无差异(P>0.05);两组临床疗效比较差异无统计学意义(P>0.05).结论:术前CT三维重建定位可精准指导上颌窦良性病变手术入路选择,提高病变完整切除率,在不降低临床总有效率的前提下,显著优化围手术期关键指标,减轻手术创伤与疼痛,降低并发症及复发率.
Objective:To explore the guiding value of benign lesions of maxillary sinus localization based on preoperative computed tomography(CT)three-dimensional reconstruction in the selection of middle nasal meatus combined with inferior nasal meatus fenestration approach and anterior lacrimal recess approach.Methods:116 patients with benign lesions of maxillary sinus admitted to our hospital from January 2021 to July 2024 were selected and divided into two groups according to the random number table method.The three-dimensional reconstruction group(n=58)underwent the middle nasal meatus combined with inferior nasal meatus fenestration approach or anterior lacrimal recess approach based on the lesion location of preoperative CT three-dimensional reconstruction,while the control group(n=58)underwent the middle nasal meatus combined with inferior nasal meatus fenestration approach or anterior lacrimal recess approach based on conventional two-dimensional CT axial/coronal imaging combined with nasal endoscopy findings.The incidence rateof adjusting approach decision during operation,complete resection rate of lesions,surgical indexes(intraoperative blood loss,operation time,hospitalization time),visual analogue scale of pain(VAS)and incidence rateof complications were compared between the two groups,and the clinical efficacy of patients was evaluated after operation,and the recurrence rate was recorded after one-year follow-up.Results:The complication rate and recurrence rate of the three-dimensional reconstruction group were lower than those of the control group(P<0.05),and the complete resection rate of the lesion was higher than that of the control group(P<0.05),and the intraoperative blood loss was less than that of the control group(P<0.05).The pain VAS scores of the two groups:3 months after operation<48 h after operation<6 h after operation<24 h after operation(P<0.05),and the pain VAS scores of the three-dimensional reconstruction group at 6 h,24 h and 48 h after operation were lower than those of the control group(P<0.05),but there was no statistical difference in the pain VAS scores of the two groups at 3 months after operation(P>0.05).There was no significant difference in clinical efficacy between the two groups(P>0.05).Conclusion:The localization of benign lesions of maxillary sinus based on preoperative CT three-dimensional reconstruction has guiding value for the selection of middle nasal meatus combined with inferior nasal meatus fenestration approach and anterior lacrimal recess approach,which canimprove the complete resection rate of lesions,reduce surgical trauma and pain,reduce complications,improve surgical efficacy and reduce postoperative recurrence,without compromising overall clinical efficacy.
