目的:整合颈动脉超声定量参数、小而密低密度脂蛋白(sd-LDL)及低密度脂蛋白(LDL)亚型,探究其与急性脑梗死患者脑动脉显著狭窄的相关性,筛选独立影响因素并构建联合诊断模型,弥补单一指标局限性.方法:选取2023年1月1日至2024年1月31日于我院就诊的347例急性脑梗死患者.依NASCET标准计算狭窄率,分为无狭窄组(n=42)、轻度狭窄(1%~29%)组(n=118)、中度狭窄(30%~69%)组(n=104)及重度狭窄(70%~99%)组(n=83);以狭窄率50%为界,将305例脑动脉狭窄患者分为显著狭窄组(n=165)与非显著狭窄组(n=140).收集一般资料、颈动脉超声参数(内膜中层厚度、斑块数量/面积)及血清学指标(sd-LDL、LDL亚型、载脂蛋白B等).采用Spearman秩相关、多因素Logistic回归分析急性脑梗死患者各指标与脑动脉狭窄的相关性和脑动脉显著狭窄的影响因素.绘制受试者工作特征曲线,评估指标对脑动脉显著狭窄的诊断效能.结果:内膜中层厚度、斑块数量、斑块面积、sd-LDL、LDL-B型与狭窄程度呈正相关(P<0.001),LDL-A型呈负相关(P<0.001),LDL-Ⅰ型呈微弱负相关(P<0.05).Logistic多因素回归分析得出,糖尿病、内膜中层厚度、斑块数量、斑块面积、载脂蛋白(Apo-B)、sd-LDL、LDL-A型、LDL-B型是脑动脉显著狭窄的独立影响因素(均P<0.05);LDL-C未进入最终回归模型,其并非脑动脉显著狭窄的独立影响因素.Hosmer-Lemeshow拟合优度检验显示,x2=6.669,P=0.573,模型拟合良好.上述指标联合预测显著狭窄的曲线下面积为0.849,灵敏度76.40%、特异度80.70%,优于单项指标诊断效能.结论:急性脑梗死患者的颈动脉超声定量参数、sd-LDL水平及LDL亚型分布与脑动脉显著狭窄密切相关,联合构建的模型诊断效能优异,可为临床早期识别高危患者提供参考.

Objective:To investigate the association of integrated carotid ultrasound quantitative parameters,small dense low-density lipoprotein(sd-LDL),and low-density lipoprotein(LDL)subfractions with significant cerebral artery stenosis in patients with acute cerebral infarction,and to identify independent influencing factors and construct a combined diagnostic model to overcome the limitations of single indicators.Methods:347 patients with acute cerebral infarction admitted to our hospital from January 1,2023,to January 31,2024 were enrolled.Stenosis rate was calculated according to the NASCET criteria.Patients were categorized into:no stenosis group(n=42),mild stenosis(1%~29%)group(n=118),moderate stenosis(30%~69%)group(n=104),and severe stenosis(70%~99%)group(n=83).Using a 50%stenosis rate as the cutoff,the 305 patients with cerebral artery stenosis were further divided into a significant stenosis group(n=165)and a non-significant stenosis group(n=140).General data,carotid ultrasound parameters(intima-media thickness,plaque number/area),and serological markers(sd-LDL,LDL subfractions,apolipoprotein B,etc.)were collected.Spearman rank correlation and multivariate Logistic analysis were used to examine the correlation between indicators and stenosis degree,and to identify influencing factors for significant cerebral artery stenosis.Receiver operating characteristic curves were plotted to evaluate the diagnostic performance of indicators for significant stenosis.Results:Intima-media thickness,plaque number,plaque area,sd-LDL,and LDL-B subtype were positively correlated with stenosis degree(all P<0.001),while LDL-A subtype was negatively correlated(P<0.001).LDL-Ⅰ subtype showed a weak negative correlation(P<0.05).Multivariate Logistic regression analysis identified diabetes mellitus,intima-media thickness,plaque number,plaque area,apolipoprotein B(Apo-B),sd-LDL,LDL-A,and LDL-B as independent influencing factors for significant cerebral artery stenosis(all P<0.05).LDL-C did not enter the final regression model,indicating it is not an independent influencing factor.The Hosmer-Lemeshow goodness-of-fit test showed x2=6.669,P=0.573,indicating good model fit.The area under the curve for the combined prediction of significant stenosis by the above indicators was 0.849,with a sensitivity of 76.40%and a specificity of 80.70%,outperforming the diagnostic efficacy of any single indicator.Conclusion:Carotid ultrasound quantitative parameters,sd-LDL levels,and LDL subfraction distribution are closely associated with significant cerebral artery stenosis in patients with acute cerebral infarction.The combined diagnostic model demonstrates excellent performance and may provide a reference for early clinical identification of high-risk patients.
