何嘉琳论治宫腔粘连的经验探析OA
HE Jialin's Experience in Treating Intrauterine Adhesions
宫腔粘连是继发不孕的主要原因之一,其核心病机为子宫内膜损伤后纤维化修复异常.全国名中医何嘉琳教授基于"妇人三因学说"和现代生殖医学理论,提出宫腔粘连"本虚标实"的病机,创新性建立"分类分期诊疗方案".轻度主张中药治本调经,中重度主张中西结合治疗.根据宫腔镜术后子宫内膜修复的三阶段对应中医"瘀热互结-气血亏虚-奇经失养"的病机演变,提出"术后急性期清热凉血止血、组织增生期补益气血以扶正、内膜重塑期填补奇经养胞宫"的诊治方案,临床疗效卓越.
Intrauterine adhesions are one of the main causes of secondary infertility,with the core pathogenesis be-ing abnormal fibrosis and repair after endometrial injury.Professor HE Jialin,a nationally renowned traditional Chinese medicine(TCM)practitioner,based on the"Three Causes of Gynecological Diseases"theory and modern reproductive medicine,proposed a"deficiency in the root and excess in the branch"pathogenesis of intrauterine adhesions and in-novatively established a"classification and staging treatment plan."For mild cases,TCM is advocated to treat the root cause and regulate menstruation;for moderate to severe cases,a combination of TCM and Western medicine is recom-mended.According to the three stages of endometrial repair after hysteroscopy,corresponding to the TCM pathogenesis of"blood stasis and heat accumulation-Qi and blood deficiency-malnourishment of the extraordinary meridians,"a treatment plan was proposed:"clearing heat,cooling blood,and stopping bleeding in the acute postoperative phase;tonifying Qi and blood to support the body's resistance in the tissue proliferation phase;and filling the extraordinary meridians to nourish the uterus in the endometrial remodeling phase."This approach has shown excellent clinical ef-ficacy.
