芒针透刺督脉组穴对脑卒中后痉挛性偏瘫患者皮质脊髓束重塑的影响OA
The Effect of Elongated Needle Penetration of Governor Vessel Acupoint Groups on Corticospinal Tract Remodeling in Post-Stroke Patients with Spastic Hemiplegia
目的 观察芒针透刺督脉组穴治疗脑卒中后痉挛性偏瘫(post-stroke spasticity,PSS)的临床疗效,并观察其对皮质脊髓束(corticospinal tract,CST)神经重塑的影响.方法 将62例PSS患者随机分为观察组和对照组,每组31例,最终完成59例(观察组30例,对照组29例).两组患者均接受基础治疗及康复训练;观察组采用芒针透刺督脉组穴治疗(主穴:百会透脑户、大椎透至阳、筋缩透悬枢、命门透腰阳关,随症配穴),每次30 min;对照组采用常规针刺治疗(主穴:水沟、内关、三阴交等,随症配穴),每次30 min;两组均每周治疗5次,10次为1个疗程,共治疗3个疗程.分别于治疗前后评定患者改良Ashworth量表(modified Ashworth scale,MAS)评分、改良Barthel指数(modified Barthel index,MBI)评分、简易 Fugl-Meyer 评估量表(Fugl-Meyer assessment,FMA)评分,并应用弥散张量成像及弥散张量纤维束示踪成像技术检测CST各向异性分数值(fractional anisotropy,FA)、双侧各向异性分数比值(bilateral fractional anisotropy ratio,rFA)及纤维束缺损等级,同时评定临床疗效.结果 两组患者治疗后MBI评分、FMA评分均较治疗前显著升高(P<0.05),MAS评分、CST缺损等级均较治疗前显著降低(P<0.05),且观察组MBI、FMA评分升高和MAS评分、CST缺损等级降低程度均较对照组更显著(P<0.05);治疗后两组患者患侧内囊后肢、大脑脚、脑桥处FA及rFA均较治疗前显著升高(P<0.05),且观察组升高程度较对照组更显著(P<0.05);观察组临床疗效显著优于对照组(P<0.05).结论 芒针透刺督脉组穴可有效改善PSS患者肌张力、运动功能及日常生活能力,其疗效优于常规针刺,其作用机制可能与促进CST神经纤维重塑有关.
Objective To investigate the clinical efficacy of elongated needle acupuncture at governor vessel acupoints in the treatment of post-stroke spastic hemiplegia(PSS),as well as its effect on neural remodeling of the corticospinal tract(CST).Methods A total of 62 PSS patients were randomly divided into observation group and control group,with 31 patientsin each group,and finally 59 patients(30 in the observation group and 29 in the control group)completed the study.Both groups re-ceived basic treatment and rehabilitation training;the patients in the observation group were treated with elongated needle acu-puncture at governor vessel acupoints(with the main points of Baihui penetrating to Naohu,Dazhui penetrating to Zhiyang,Jin-suo penetrating to Xuanshu,and Mingmen penetrating to Yaoyangguan,supplemented by acupoints based on syndrome differ-entiation)for 30 minutes each time,and those in the control group were treated with conventional acupuncture(with the main points including Shuigou,Neiguan,and Sanyinjiao,supplemented by acupoints based on syndrome differentiation)for 30 mi-nutes each time;both groups were treated 5 times a week,with 10 times as one course of treatment,and both groups were treated for 3 courses.The patients were assessed based on Modified Ashworth Scale(MAS)grade,Modified Barthel Index(MBI)score,and Fugl-Meyer Assessment(FMA)score before and after treatment,and diffusion tensor imaging and diffusion tensor tractography were used to measure the fractional anisotropy(FA),relative fractional anisotropy(rFA)ratio,and fiber tract defect of the CST.Clinical outcome was also assessed.Results After treatment,both groups had significant increases in FMA and MBI scores(P<0.05)and significant reductions in MAS score and CST fiber tract defect(P<0.05),and compared with the control group,the observation group had significantly greater changes(P<0.05).After treatment,both groups had significant increases in the FA and rFA values of the posterior limb of the internal capsule,the cerebral peduncle,and the pons on the affected side(P<0.05),and the observation group had significantly greater increases than the control group(P<0.05).The observation group had a significantly better clinical outcome than the control group(P<0.05).Conclusion Elon-gated needle acupuncture at governor vessel acupoints has a better clinical efficacy than conventional acupuncture and can effec-tively improve muscle tone,motor function,and activities of daily living in PSS patients,possibly by promoting the neural re-modeling of the CST.
