目的 探讨血小板参数联合临床指标对早产儿支气管肺发育异常(bronchopulmonary dysplasia,BPD)的预测价值.方法 回顾性分析2020年1月至 2025年1月合肥市妇幼保健院收治的早产儿272例.根据BPD诊断标准将其分为BPD组(n=133)与非BPD组(n=139).比较两组早产儿的一般临床资料及生后第1天、第1周、第2周、第3周的血小板计数(platelet count,PLT)、血小板体积分布宽度、血小板压积、平均血小板体积的差异,采用二分类Logistic回归分析及受试者操作特征曲线评估临床特征联合血小板相关参数对BPD的预测价值.结果 二分类Logistic回归分析显示胎龄(OR=0.93)、第3周PLT(OR=0.99)、肺表面活性物质(pulmonary surfactant,PS)应用(OR=2.41)、咖啡因使用时间(OR=1.03)均是早产儿发生BPD的独立影响因素(P<0.05).四者联合可预测BPD的发生,曲线下面积为0.802,敏感度为79.7%,特异性为68.3%.结论 第3周PLT降低是早产儿发生BPD的独立危险因素,联合胎龄、有无PS应用、咖啡因使用时间对早产儿BPD的发生有一定的预测价值.

Objective To investigate the predictive value of platelet parameters combined with clinical indicators for bronchopulmonary dysplasia(BPD)in premature infants.Methods A retrospective analysis was conducted on 272 premature infants admitted to Hefei Maternal and Child Health Hospital from January 2020 to January 2025.According to the BPD diagnostic criteria,they were divided into BPD group(n=133)and non-BPD group(n=139).General clinical data and platelet parameters,including platelet count(PLT),platelet distribution width,plateletcrit,and mean platelet volume,measured on postnatal day 1 and at weeks 1,2,and 3,were compared between two groups.Binary Logistic regression and receiver operating characteristic curve were used to evaluate the predictive value of clinical characteristics combined with platelet-related parameters for BPD.Results Binary Logistic regression analysis identified gestational age(OR=0.93),PLT at week 3(OR=0.99),pulmonary surfactant(PS)administration(OR=2.41),and duration of caffeine therapy(OR=1.03)as independent predictors of BPD(P<0.05).The combination of these four factors predicted BPD with an area under the curve of 0.802,a sensitivity of 79.7%,and a specificity of 68.3%.Conclusion Reduced PLT at the third week is an independent risk factor for BPD in premature infants.Its combination with gestational age,PS administration,and duration of caffeine therapy provides certain predictive value for the occurrence of BPD.
