目的 探讨O6-甲基鸟嘌呤-DNA甲基转移酶(O6-methylguanine-DNA methyltransferase,MGMT)联合存活素对食管癌患者新辅助放化疗敏感性的预测效能.方法 回顾性选择2021年1月至2023年12月陕西省人民医院收治的接受新辅助放化疗前的cⅡ~Ⅳa期食管癌患者154例,根据放化疗疗效分为有效组(89例)和无效组(65例).比较两组患者放化疗前的临床资料及肿瘤组织中MGMT、存活素mRNA表达水平.采用logistic回归分析筛选新辅助放化疗疗效的影响因素.采用ROC曲线分析各单一指标及经logistic回归拟合的联合指标对新辅助放化疗疗效的预测效能.结果 无效组患者的肿瘤T分期、N分期、组织学分级及MGMT、存活素mRNA表达水平均显著高于有效组(均P<0.05).logistic回归分析显示,T分期、组织学分级及MGMT、存活素mRNA表达水平均为食管癌新辅助放化疗疗效的独立影响因素(均P<0.05).ROC曲线分析显示,T分期、组织学分级、MGMTmRNA、存活素mRNA单独预测新辅助放化疗疗效的AUC分别为0.624、0.571、0.763、0.787.四者联合预测的AUC为0.871,显著高于各单一指标,联合预测的灵敏度为0.800、特异度为0.820.结论 食管癌患者MGMT、存活素mRNA表达水平升高与新辅助放化疗无效密切相关.T分期、组织学分级联合MGMT、存活素mRNA表达水平对食管癌新辅助放化疗疗效具有较好的预测效能,可为临床个体化治疗提供参考.

Objective To explore the predictive efficacy of O6-methylguanine-DNA methyltransferase(MGMT)combined with survivin in predicting the sensitivity of neoadjuvant chemoradiotherapy in patients with esophageal cancer.Methods A total of 154 patients with stage cⅡ-Ⅳa esophageal cancer who received neoadjuvant chemoradiotherapy and were admitted to Shaanxi Provincial People's Hospital from January 2021 to December 2023 were retrospectively enrolled,and divided into the effective group(89 cases)and the ineffective group(65 cases)according to the curative effect of chemoradiotherapy.The clinical data and the mRNA expression levels of MGMT and survivin before chemoradiotherapy were compared between the two groups.Logistic regression analysis was used to screen the influencing factors of neoadjuvant chemoradiotherapy efficacy,and ROC curve analysis was adopted to evaluate the predictive efficiency of each single index and the combined index fitted by logistic regression on the efficacy of neoadjuvant chemoradiotherapy.Results The tumor T stage,N stage,histological grade,and mRNA expression levels of MGMT and survivin in the ineffective group were significantly higher than those in the effective group(all P<0.05).Logistic regression analysis showed that T stage,histological grade,and mRNA expression levels of MGMT and survivin were all independent influencing factors of neoadjuvant chemoradiotherapy efficacy for esophageal cancer(all P<0.05).ROC curve analysis revealed that the AUC of T stage,histological grade,MGMT mRNA and survivin mRNA alone in predicting neoadjuvant chemoradiotherapy efficacy was 0.624,0.571,0.763 and 0.787,respectively;the AUC of combined prediction of the four indicators was 0.871,which was significantly higher than that of each single indicator,with a sensitivity of 0.800 and a specificity of 0.820.Conclusion The elevated expression of MGMT and survivin in esophageal cancer patients is closely associated with the ineffectiveness of neoadjuvant chemoradiotherapy.T stage,histological grade combined with mRNA expression levels of MGMT and survivin present favorable predictive efficacy for the efficacy of neoadjuvant chemoradiotherapy in esophageal cancer,which can provide a reference for clinical individualized treatment.
