骨化三醇抑制试验在维生素D缺乏相关正常血钙甲状旁腺功能亢进症筛查中的应用OA
Application of the calcitriol suppression test in screening for normocalcemic hyperparathyroidism and vitamin D deficiency
目的 评估骨化三醇抑制试验筛查诊断维生素 D 缺乏但血钙正常的原发性与继发性甲状旁腺功能亢进症的应用价值.方法 对 2023 年 7 月至 2024 年 12 月在新疆医科大学第一附属医院门诊就诊的甲状旁腺激素(parathyroid hormone,PTH)水平升高、血钙水平正常或轻度升高、维生素 D 缺乏的患者 43 例,进行口服骨化三醇抑制试验,给予骨化三醇 0.25 μg、3 次/d 口服 1 周,完善试验前后 25 羟维生素 D[25-hydroxyvitamin D,25(OH)D]、PTH、甲状旁腺超声等指标,计算 PTH 抑制比(PTH-IR).采用受试者工作特征(receiver operating characteristic,ROC)曲线评估骨化三醇抑制试验筛查诊断的应用价值.结果 共纳入43 例受试者,按照原发性甲状旁腺功能亢进症诊断标准,分为合并维生素 D 缺乏的正常血钙原发性甲状旁腺功能亢进症(normocalcemic primary hyperparathyroidism,NPHPT)组9 例和维生素 D 缺乏导致的继发性甲状旁腺功能亢进症(secondary hyperparathyroidism,SHPT)组34 例.与SHPT 组相比,NPHPT 组PTH-IR 比值显著升高[0.78(0.7,1.0)vs.1.20(0.9,1.5),P<0.05].NPHPT 组中PTH-IR>1 者居多(7/9),而SHPT组中 PTH-IR<1 者居多(29/34).通过 ROC 曲线评估,PTH-IR 诊断 NPHPT 患者的曲线下面积(area under the curve,AUC)为(0.758,95%CI:0.557~0.960).PTH-IR 诊断 NPHPT 的灵敏度、特异度、阳性预测值、阴性预测值分别为 77.8%、85.3%、58.3%、93.5%.结论 骨化三醇抑制试验用于筛查 NPHPT 的诊断性功能试验,简便可行,具有较好的敏感性和特异性,有待于用于多中心、大样本量进一步研究.
Objective To evaluate the application value of the calcitriol suppression test as a screening tool for primary hyperparathyroidism(NPHPT)and secondary hyperparathyroidism(SHPT)caused by vitamin D deficiency in patients with normal serum calcium levels.Methods From July 2023 to December 2024,43 patients with elevated para-thyroid hormone(PTH)levels,normal or mildly elevated serum calcium levels,and vitamin D deficiency were recruited from the outpatient department of the First Affiliated Hospital of Xinjiang Medical University.All patients underwent an oral calcitriol suppression test,receiving 0.25 μg of calcitriol three times daily for one week.Pre-and post-test levels of 25-hydroxyvitamin D[25(OH)D],PTH,and ultrasonographic findings of the parathyroid gland were recorded.The PTH suppression ratio was calculated.The diagnostic value of the calcitriol suppression test was assessed using receiver operat-ing characteristic(ROC)curve analysis.Results According to diagnostic criteria for primary hyperparathyroidism,par-ticipants were divided into two groups:normocalcemic primary hyperparathyroidism with vitamin D deficiency(NPHPT group,n=9)and SHPT caused by vitamin D deficiency(SHPT group,n=34).Compared to the SHPT group,the NPHPT group showed a significantly higher PTH-IR ratio[0.78(0.7,1.0)vs.1.20(0.9,1.5),P<0.05].The ma-jority of patients in the NPHPT group had PTH-IR>1(7/9),whereas most patients in the SHPT group had PTH-IR<1(29/34).ROC curve analysis showed that the area under the curve(AUC)for PTH-IR in diagnosing NPHPT was 0.758(95%CI:0.557-0.960).The sensitivity,specificity,positive predictive value,and negative predictive value of PTH-IR for diagnosing NPHPT were 77.8%,85.3%,58.3%,and 93.5%,respectively.Conclusion The calcitriol suppression test is a simple and feasible diagnostic test for screening NPHPT,with good sen-sitivity and specificity.Further research in multicenter and large-sample studies is needed.
