50岁以上骨质疏松性骨折患者血清尿酸与全身炎症反应指数的相关性OA
Correlation between serum uric acid levels and systemic inflammation response index in osteoporotic fracture patients over 50 years old
目的 探讨老年骨质疏松性骨折(osteoporotic fractures,OPF)患者血清尿酸(serum uric acid,SUA)与全身炎症反应指数(systemic inflammation response index,SIRI)之间的关系,并评估体质量指数(body mass index,BMI)对该关系的调节作用.方法 本研究为一项基于江苏大学附属昆山医院 2017 年 1 月至2022 年7 月住院患者的横断面研究.纳入2 279 名50 岁以上、经诊断为原发性OPF 的住院患者.SUA 为暴露变量,SIRI 为结局变量,协变量包括人口学因素、代谢指标及血常规参数等.采用广义估计方程(generalized estimating equations,GEE)和广义加性模型(generalized additive model,GAM)评估 SUA 与 SIRI之间的线性与非线性关系,并进行 BMI 分层分析与两段式回归建模.结果 SUA 与 SIRI 呈显著正相关(β=0.528,95%CI:0.294~0.761,P<0.001),该关系在 BMI 24~28 kg/m2 组中最为显著,存在明显拐点效应(P=0.001).当 SUA 水平低于拐点(415 μmol/L)时,与 SIRI 呈正相关,高于该阈值后关联减弱.结论 SUA 与SIRI 之间存在 BMI 依赖的非线性关系,提示其可能作为炎症风险提示指标.
Objective To investigate the association between serum uric acid(SUA)and the systemic inflam-mation response index(SIRI)in elderly adults with osteoporotic fracture(OPF),and to explore the modifying role of body mass index(BMI)in this relationship.Methods This cross-sectional study included 2 279 hospitalized patients aged≥50 years with primary OPF admitted to Kunshan Hospital of Jiangsu University between January 2017 and July 2022.SUA was treated as the exposure variable and SIRI as the outcome variable.Covariates included demographic fac-tors,metabolic parameters,and hematological parameters.Generalized estimating equations(GEE)and generalized addi-tive models(GAM)were used to assess linear and nonlinear associations between SUA and SIRI.Stratified analyses and two-piecewise linear regression were performed based on BMI categories.Results SUA was significantly positively associ-ated with SIRI(β=0.528,95%CI:0.294-0.761,P<0.001).This association was strongest in the BMI 24-28 kg/m2 group,with a clear threshold effect(P=0.001).When SUA levels were below the inflection point(415 μmol/L),SUA was positively correlated with SIRI;above this threshold,the association was attenuated.Conclusion There is a BMI-de-pendent nonlinear relationship between SUA and SIRI,suggesting its potential role as an indicator of inflammation risk.
