老年难治性胃食管反流病是消化科疑难疾病,单纯西药抑酸治疗效果欠佳.本文系统阐释老年难治性胃食管反流病"中焦虚损,枢机失序"的核心病机,提出病势演变呈现"虚、郁、瘀、毒"之间的动态转化规律.中焦虚损致气机升降逆乱,肝郁痰阻酿成气机壅滞,久病入络引发虚瘀互结,浊毒蓄积终致形质损伤.针对此病机特点,确立培土固本、疏瀹气机、和血通络、化浊解毒、三焦分消五维治疗体系,为中西医协同防治难治性胃食管反流病提供了理论依据与临床路径.未来研究可围绕"虚、郁、瘀、毒"的核心生物学标志物深入探索,以推动该诊疗体系的精准应用.

Elderly refractory gastroesophageal reflux disease(RGERD)is a challenging disorder in gastroenterology,and treatment with acid suppression alone often yields unsatisfactory outcomes.This paper systematically elucidates the core pathogenesis of elderly RGERD as"deficiency of the middle Jiao and disorder of the pivotal mechanism".It proposes that the disease evolution follows a dynamic transformation pattern among"deficiency,stagnation,stasis,and toxin".Deficiency of the middle Jiao leads to ascending and descending derangement of Qi movement;liver Qi stagnation with phlegm obstruction generates Qi stagnation;chronic disease invading the collaterals results in simultaneous deficiency and stasis;and accumulation of turbid toxin eventually causes structural damage.Based on this pathogenetic characteristic,a five-dimensional therapeutic system is established,including reinforcing the earth and consolidating the root,dredging and rectifying Qi movement,harmonizing blood and unblocking the collaterals,resolving turbidity and detoxifying,and treating via Sanjiao differentiation.This provides a theoretical basis and clinical pathway for integrated traditional Chinese and Western medicine management of RGERD.Future research may focus on the core biological signatures of"deficiency,stagnation,stasis,and toxin"to further refine the precise application of this diagnostic and therapeutic system.
