文章探讨在脉冲电场消融(PFA)时代,Marshall静脉(VOM)无水酒精消融在二尖瓣峡部(mitral isthmus,MI)消融中的应用价值.分析MI阻滞对心房颤动(房颤)治疗的重要性,阐述射频消融联合VOM无水酒精消融的协同机制与临床获益,分析当前双极PFA在MI消融中存在的作用深度不足及长期疗效局限等问题,指出在现阶段,联合消融仍是实现MI持久阻滞更为可靠的策略,并展望了单极PFA技术及多能量整合的未来发展方向.

The article explores the clinical value of vein of Marshall(VOM)ethanol ablation in mitral isthmus ablation in the new era of pulsed field ablation(PFA)for atrial fibrillation.It analyzes the importance of mitral isthmus block in atrial fibrillation treatment,discusses the synergistic mechanism and clinical benefits of radiofrequency ablation combined with VOM,and analyzes the insufficient ablation depth and limited long-term effect of current bipolar PFA in mitral isthmus ablation.It is pointed out that combined ablation is still a more reliable strategy for achieving persistent block of MI at this stage.The future development direction is prospected in unipolar PFA technique and the multi-energy integration.
