脉冲电场消融(PFA)作为一种非热能的组织选择性消融技术,已成为心房颤动(房颤)导管消融领域革命性的新平台.其通过高压脉冲电场不可逆电穿孔,致使心肌细胞凋亡,而周围组织如食管、血管、神经等得以最大程度保留.这一独特机制使得PFA在高效实现肺静脉隔离的同时,显著降低了传统热消融(射频、冷冻)相关的并发症风险.然而,新技术的引入也带来了新的、特有的潜在风险,如急性溶血、冠脉痉挛及亚临床脑损伤等.因此,PFA的围手术期管理不仅需遵循常规原则,更需针对PFA的物理特性进行精细化调整.文章旨在结合最新证据与临床经验,系统阐述房颤PFA围手术期管理的全流程要点,并特别聚焦于其特有并发症的监测与防治,以促进该技术的安全、规范应用.
Pulsed field ablation(PFA),as a non-thermal,tissue-selective ablation technique,has emerged as a revolutionary novel platform in the field of catheter ablation for atrial fibrillation.It induces irreversible electroporation via high-voltage pulsed electric fields,leading to cardiomyocyte apoptosis,while adjacent tissues such as the esophagus,blood vessels,and nerves are preserved to the maximum extent possible.This unique mechanism enables PFA to achieve efficient pulmonary vein isolation(PVI)while significantly reducing the risk of complications associated with conventional thermal ablation modalities(i.e.,radiofrequency and cryoablation).Nevertheless,the introduction of this novel technology has also brought about new and unique potential risks,including acute hemolysis,coronary artery spasm,and subclinical cerebral injury.Therefore,perioperative management of PFA must not only adhere to routine principles but also be subject to refined adjustments tailored to the physical characteristics of PFA.Based on the latest evidence and clinical experience,this article aims to systematically elaborate on the key aspects of the entire process of perioperative management of PFA in atrial fibrillation,with a particular focus on the monitoring,prevention,and treatment of its specific complications,so as to promote the safe and standardized application of this technology.
