Application of pulsed field ablation in the treatment of atrial fibrillation:from theoretical exploration to clinical practice
心房颤动(房颤)作为最常见的快速性心律失常,其发病率随人口老龄化逐年升高,给患者和社会带来了沉重的疾病负担.导管消融是房颤节律控制的重要手段,传统射频消融虽应用广泛,但存在蒸汽炸裂、毗邻组织损伤等固有缺陷.脉冲电场消融(PFA)作为一种非温度依赖的新型消融技术,凭借心肌组织选择性电穿孔特性,在减少并发症方面展现出独特优势.文章系统梳理了 PFA的技术原理、早期基础实验与临床探索、大规模真实世界研究数据,重点分析其在阵发性及持续性房颤治疗中的有效性与安全性,并探讨当前面临的冠脉损伤、器械异质性、额外消融策略等关键问题,展望PFA在三维导航优化、术式创新、术后管理等领域的发展方向,以期为临床实践与科研探索提供参考.
Atrial fibrillation(AF)is the most common tachyarrhythmia,with its incidence increasing year by year with population aging,imposing a heavy disease burden on patients and society.Catheter ablation is an important method for rhythm control of AF.Although traditional radiofrequency ablation is widely used,it has inherent defects such as steam pop and adjacent tissue injury.As a novel non-temperature-dependent ablation technology,pulsed field ablation(PFA)exhibits unique advantages in reducing complications due to its characteristic of selective electroporation of myocardial tissue.This article systematically reviews the technical principles,early basic experiments,clinical exploration,and large-scale real-world research data of PFA,focusing on analyzing its efficacy and safety in the treatment of paroxysmal and persistent AF.It also discusses the current key issues such as coronary artery injury,device heterogeneity,and additional ablation strategies,and finally looks forward to the development direction of PFA in the fields of optimization of three-dimensional navigation,innovation of surgical mode,and postoperative management,aiming to provide references for clinical practice and scientific research.
