儿科内分泌遗传代谢病领域诊疗进展与展望OA
Advances and prospects in diagnosis and treatment of pediatric endocrinology,genetics and metabolism
随着新技术的不断进展,我国儿科内分泌遗传代谢病领域正经历从传统表型描述向分子分型、从激素替代向靶向干预、从单一诊疗向健康管理的深刻转型.该文围绕"精确诊断、靶向治疗、系统管理"三个关键词,梳理了五个常见内分泌疾病领域中的争议、进展以及展望.1型糖尿病领域,生物标志物与人工智能模型助力早期识别,替利珠单抗代表着免疫干预新方法.生长与骨骼疾病方面,分子通路分类重塑诊断格局,抗FGF23单抗等靶向药物实现从磷酸盐替代到源头干预的跨越,但新药的长期获益仍缺乏.性发育异常的诊疗正从染色体核型走向基因网络解析,基因编辑与类器官技术带来希望却也面临伦理挑战,临床争议集中于手术时机、心理干预模式,新治疗有待突破.儿童肥胖诊疗转向多维分型与系统性分层干预,但生活方式干预仍是基石,靶向药物为特定患儿提供新选择.甲状腺疾病精准管理向多维度评估发展,靶向药物与风险分层工具为个体化治疗提供新方向.但是新技术新方法的应用仍需进一步研究验证,未来需在真实世界研究、伦理监管及多学科协作等方面持续探索.
With the continuous advancement of new technologies,the field of pediatric diseases of endocrinology and genetic metabolism in China is undergoing a profound transformation,shifting from traditional phenotypic descriptions to molecular subtyping,from hormone replacement to targeted interventions,and from single-dimension diagnosis and treatment to integrated health management.Centered on the three key concepts of"precision diagnosis,targeted therapy and systematic management",this article reviews the controversies,progress,and future perspectives in five common endoc rine disorders.In the field of type 1 diabetes mellitus,biomarkers and artificial intelligence models facilitate early identification,while teplizumab represents a novel approach to immune intervention.In growth and skeletal disorders,molecular pathway-based classification is reshaping the diagnostic landscape,and targeted therapies such as anti-FGF23 monoclonal antibodies have achieved a transition from replacement to source intervention;however,the long-term benefits of novel drugs are still in lack.The diagnosis and treatment of disorders of sex development are evolving from chromosomal karyotyping to network-based genetic analysis;while gene editing and organoid technologies offer promise,they also present ethical challenges,with clinical controversies centering on the timing of surgical intervention and models of psychological intervention,and new therapies are expected to have breakthroughs.The diagnosis and management of childhood obesity is shifting toward multi-dimensional phenotyping and systematic stratified intervention,but lifestyle modification remains the cornerstone,and targeted pharmacotherapies provide new options for specific child patients.Precision management of thyroid diseases is advancing toward multi-dimensional assessment,with targeted drugs and risk stratification tools offering new directions for individualized treatment.However,the application of new technologies and methods still requires further research and validation.Future efforts should focus on real-world studies,ethical governance,and multidisciplinary collaboration.
