全身振动训练结合常规康复训练对膝前痛的疗效研究OA

目的 观察全身振动训练结合常规康复训练对膝前痛患者的疗效,为膝前痛的治疗提供安全、高效的康复方案.方法 选取在国家体育总局体育医院就诊的40例膝前痛患者,随机分为对照组与试验组,每组20例.对照组进行常规康复训练,试验组进行全身振动训练结合常规康复训练,单次50min,每周3次(训练至少隔1天进行1次),训练持续8周,入组和干预8周后评估疼痛、膝前痛、生活质量、膝关节在60°/s和180°/s下等速屈伸运动测试的相对峰力矩及本体感觉.结果 两组组内比较,VAS值(P<0.01)、AKPS分值(P<0.01)、SF-36量表中躯体功能、生理职能、躯体疼痛、活力4个方面分值(P<0.01)、膝关节屈伸肌力(P<0.01)干预后较干预前存在显著性差异;本体感觉试验组干预后较干预前具有显著性差异(P<0.01),但对照组无显著性差异(P>0.05).两组组间比较,试验组与对照组仅膝关节本体感觉角度差值改善情况存在显著性差异(P<0.01).结论 全身振动训练结合常规康复训练与单独的常规康复训练都对膝前痛患者的膝关节疼痛、膝关节功能、生活质量和膝关节的屈伸肌肉力量均有显著性改善,除本体感觉外两种干预方案不存在显著性的差异.

Objective To examine the efficacy of whole-body vibration training combined with conventional rehabilitation train-ing on knee joints in individuals with anterior knee pain,and to provide a safe and efficient rehabilitation program for the treatment of anterior knee pain.Methods A total of 40 patients with anterior knee pain were randomly divided into two groups:a control group receiving conventional rehabilitation training(n=20),and an experimental group receiving whole-body vibration training combined with conventional rehabilitation training(n=20).Each training session lasted 50 minutes,conducted 3 times per week(with at least one rest day between sessions),for a total duration of 8 weeks.Before and after the intervention,outcomes including pain intensity,knee function(Anterior Knee Pain Scale,AKPS),quality of life(SF-36 scale),knee joint peak torque at 60°/s and 180°/s,and knee proprioception were assessed.Results Compared with pre-intervention,the experimental group showed significant differences in VAS scores(P<0.01),AKPS scores(P<0.01),scores in the physical functioning,role-physical,bodily pain,and vitality do-mains of the SF-36(P<0.01),knee flexor and extensor peak torque(P<0.01),and knee proprioception(P<0.01).The control group also showed significant improvements in the above indicators(P<0.01),except for proprioception,which did not change sig-nificantly(P>0.05).Between-group comparisons revealed that only knee joint proprioceptive angle difference improved more signif-icantly in the experimental group than in the control group(P<0.01).Conclusion Both whole-body vibration training combined with conventional rehabilitation training and conventional rehabilitation training alone can significantly improve knee pain,knee func-tion,quality of life,and knee flexor and extensor muscle strength in individuals with anterior knee pain.However,there was no signifi-cant difference between the two intervention regimens in most outcome measures except for proprioception.
