APTw及DCE-MRI对Ⅰ期子宫内膜癌和子宫内膜息肉的鉴别价值OA
The value of using APTw imaging and DCE-MRI to differentiate stage Ⅰ endometrial carcinoma from endometrial polyps
目的:探讨酰胺质子转移加权(APTw)成像及动态增强磁共振成像(DCE-MRI)对Ⅰ期子宫内膜癌(EC)和子宫内膜息肉(EP)的鉴别价值.材料与方法:回顾性分析 2019 年 4 月—2022 年 1 月行 3.0T磁共振扫描并经手术病理证实的 36 例Ⅰ期EC及 19 例EP,扫描序列包括矢状位APTw及DCE-MRI(40 期).记录患者临床信息,测量 3.5 ppm不对称性磁化转移率(ΔMTRasym(3.5 ppm))值和容积转移分数(Ktrans)、速率常数(Kep)、血管外细胞外间隙容积比(Ve)及血浆容积分数(Vp)值.比较EC和EP组间临床信息、定量参数间差异,使用ROC曲线评价各参数诊断效能;通过二元Logistic回归建立联合诊断模型.使用Delong检验比较独立参数和联合模型曲线下面积(AUC)间差异.采用相关性系数分析两序列差异参数间的相关性.结果:两组患者年龄、绝经状态、子宫异常出血(AUB)、人附睾蛋白 4(HE4)差异有统计学意义(P<0.05),病灶位置、形态、常规序列信号及CA125 无差异(P均>0.05).EC组ΔMTRasym(3.5 ppm)值、Ktrans、Kep 和Vp 值均高于EP组,EC组Ve 值低于EP组.ΔMTRasym(3.5 ppm)值和 Kep 是鉴别 EC 和 EP 的独立因素(P<0.05),模型的 AUC(0.937)高于单独使用 ΔMTRasym(3.5 ppm)值 AUC(0.775)(P=0.008),与Kep 值AUC(0.917)的差异无统计学意义(P=0.256).ΔMTRasym(3.5 ppm)值、Kep值AUC无差异(P=0.052).ΔMTRasym(3.5 ppm)值与Ktrans、Kep 值呈弱相关性,r值为 0.392、0.417,P值为 0.022、0.002.结论:APTw和DCE-MRI均能有效地鉴别EC和EP,两者联合能提高鉴别效能,有很好的临床应用前景.
Objective:To investigate the diagnostic value of amide proton transfer-weighted(APTw)imaging and dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in differentiating stage Ⅰ endometrial cancer(EC)from endometrial polyps(EP).Materials and Methods:A retrospective analysis was conducted on 36 patients with stage Ⅰ EC and 19 patients with EP confirmed by surgical pathology who underwent 3.0T MRI scans between April 2019 and January 2022.The scanning sequences included sagittal APTw and DCE-MRI(40 phases).Clinical information of the patients was recorded,and ΔMTRasym(3.5 ppm)value and DCE-MRI parameters(Ktrans,Kep,Ve,and Vp)were measured.Differences in clinical information and quan-titative parameters between the EC and EP groups were compared.The diagnostic performance of each parameter was evaluat-ed using receiver operating characteristic(ROC)curves.A combined diagnostic model(Model)was established using binary Lo-gistic regression.The area under the curve(AUC)differences between independent parameters and the combined model were compared using the DeLong test.The correlation between the differential parameters of the two sequences was analyzed using the correlation coefficient.Results:There were statistically significant differences in age,menopausal status,abnormal uterine bleeding(AUB),and human epididymis protein4(HE4)levels between the two groups(P<0.05).No significant differences were found in lesion location,shape,signal on conventional sequences,or CA125 levels(P>0.05 for all).The ΔMTRasym(3.5 ppm),Ktrans,Kep,and Vp values were higher in the EC group than in the EP group,while the Ve value was lower in the EC group.ΔMTRasym(3.5 ppm)value and Kep were independent factors for differentiating EC from EP(P<0.05).The AUC of the combined Model(0.937)was higher than that of ΔMTRasym(3.5 ppm)alone(0.775,P=0.008),but there was no significant difference in AUC between the Model and Kep(0.917,P=0.256).There was no significant difference between the AUCs of ΔMTRasym(3.5 ppm)and Kep(P=0.052).ΔMTRasym(3.5 ppm)showed weak correlation with Ktrans and Kep,with correlation coefficients(r)of 0.392 and 0.417,respectively(P=0.022 and P=0.002).Conclusion:Both APTw and DCE-MRI can effectively differentiate EC from EP,and their combination can improve diagnostic efficacy,showing good clinical application prospects.
