目的:探讨二维斑点追踪技术(2D-STI)对急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后左心室功能恢复情况的评估价值.方法:选取 2024 年 5 月—2025 年 6 月本院进行PCI术的AMI患者共 80 例作为观察组,于手术前后分别进行心脏彩超和 2D-STI并记录相关影像参数.检测血清N末端脑钠肽前体(NT-proBNP)及高敏肌钙蛋白T(hs-cTnT)水平.并纳入同期健康人群 80 例作为对照组.对比两组影像学参数及血清指标差异,观察AMI患者手术前后影像学参数及血清指标变化趋势,并分析 2D-STI参数与常规心脏彩超参数和血清指标相关性.结果:观察组心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)明显高于对照组(P<0.05),而左心室射血分数(LVEF)和每搏量(SV)明显低于对照组(P<0.05);观察组血清NT-proBNP、hs-cTnT显著高于对照组(P<0.05);观察组心肌节段(基底段、中间段、心尖段)纵向收缩期峰值应变(GLS)、径向收缩期峰值应变(GRS)、圆周收缩期峰值应变(GCS)、左心室基底部旋转角度峰值(Rotbase)、左心室心尖部旋转角度峰值(Rotapex)和左心室整体扭转角度峰值(Rot)绝对值明显低于对照组(P<0.05).观察组PCI术后 1 周、3 月影像参数及血清指标均较术前明显改善(P<0.05).各节段GLS、GRS、GCS、Rotbase、Rotapex、Rot绝对值与LVEDV、LVESV、NT-proBNP、hs-cTnT呈负相关,与LVEF和SV呈正相关(P<0.05).结论:2D-STI技术可有效评估AMI患者PCI术后左心室功能恢复情况,为临床诊疗提供可靠的可量化依据.

Objective:To explore the evaluation value of two-dimensional speckle-tracking imaging(2D-STI)for the recov-ery of left ventricular function in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods:A total of 80 patients with AMI undergoing PCI in the hospital were enrolled as observation group between May 2024 and June 2025,and all underwent cardiac color Doppler ultrasound and 2D-STI examinations before and after surgery.The relevant imaging parameters were recorded.The levels of serum N-terminal pro-brain natriuretic peptide(NT-proBNP)and high-sensitivity cardiac troponin T(hs-cTnT)were detected.A total of 80 healthy individuals during the same period were en-rolled as control group.The differences in imaging parameters and serum indexes between the two groups were compared.The change trends of imaging parameters and serum indexes before and after surgery in AMI patients were observed,and the cor-relation between 2D-STI parameters and routine cardiac color Doppler ultrasound parameters,serum indexes was analyzed.Results:The left ventricular end-diastolic volume(LVEDV)and left ventricular end-systolic volume(LVESV)in observation group were significantly higher than those in control group(P<0.05),while left ventricular ejection fraction(LVEF)and stroke volume(SV)were significantly lower than those in control group(P<0.05).The levels of serum NT-proBNP and hs-cTnT in ob-servation group were significantly higher than those in control group(P<0.05).The absolute values of global longitudinal strain(GLS),global radial strain(GRS)and global circumferential strain(GCS)at different myocardial segments(basal segment,middle segment,apical segment),peak rotation angle at the base of left ventricle(Rotbase),peak rotation angle at apex of left ventricle(Rotapex)and overall rotation angle peak of left ventricle(Rot)in observation group were significantly lower than those in control group(P<0.05).At 1 week and 3 months after PCI,imaging parameters and serum indexes were significantly improved in ob-servation group(P<0.05).The absolute values of GLS,GRS,GCS,Rotbase,Rotapex and Rot in different segments were negatively correlated with LVEDV,LVESV,NT-proBNP and hs-cTnT,while positively correlated with LVEF and SV(P<0.05).Conclu-sion:2D-STI can effectively evaluate the recovery of left ventricular function in AMI patients after PCI,which provides reli-able and quantifiable evidence for clinical diagnosis and treatment.
