目的:探究定量动态增强磁共振成像(DCE-MRI)定量参数对乳腺癌患者的预测效能及与血管生成素 2、血管内皮生长因子的相关性.方法:选取我院 2022 年 4 月—2025 年 4 月收治的 130 例乳腺癌患者为疾病组,选取同期、同年龄段 110 例健康志愿者为对照组,对比两组及乳腺癌不同临床分期下DCE-MRI定量参数,并通过绘制受试者工作特征(ROC)曲线分析DCE-MRI定量参数对乳腺癌患者的预测效能.结果:较对照组,实验组血管内皮生长因子、血管外细胞外容积分数(Ve)、容积转移常数(Ktrans)、速率常数(Kep)、血管生成素 2 均上升(P<0.05).较Ⅰ期比,Ⅱ、Ⅲ、Ⅳ期血管内皮生长因子、Ve、Ktrans、血管生成素 2、Kep 提升;较Ⅱ期,血管内皮生长因子、Ve、Ktrans、血管生成素 2、Kep 在Ⅲ、Ⅳ期中上升;与Ⅲ期,Ⅳ期血管内皮生长因子、Ve、Ktrans、血管生成素 2、Kep 升高显著(P<0.05).Pearson相关性分析,Ve、Ktrans、Kep 与血管生成素 2、血管内皮生长因子呈正相关(P<0.05).ROC曲线表示Ve、Ktrans、Kep 联合检测的预测效能高于单项预测(P<0.05).结论:Ve、Ktrans、Kep 在乳腺癌患者中均升高,并与血管生成素 2、血管内皮生长因子水平呈正相关,同时Ve、Ktrans、Kep 联合检测对乳腺癌的预测效能较高,临床可通过对乳腺癌患者进行DCE-MRI检查,从而为临床初步评估患者病情及后续治疗提供参考.

Objective:To analyze the predictive efficacy of DCE-MRI quantitative parameters in breast cancer patients and their correlation with angiopoietin 2 and vascular endothelial growth factor.Methods:A total of 130 breast cancer patients treated in our hospital from April 2022 to April 2025 were selected as the disease group,and 110 healthy volunteers of the same period and age were selected as the control group.The quantitative parameters of DCE-MRI in the two groups and in different clinical stages of breast cancer were compared.The predictive efficacy of DCE-MRI quantitative parameters for breast cancer patients was analyzed by constructing receiver operating characteristic(ROC)curves.Results:Compared with the control group,vascular endothelial growth factor,Ve,Ktrans,angiopoietin 2,and Kep were increased in the disease group(P<0.05).Compared with stage Ⅰ,vascular endothelial growth factor,Ve,Ktrans,angiopoietin 2,and Kep were increased in stagesⅡ,Ⅲ,and Ⅳ;compared with stage Ⅱ,vascular endothelial growth factor,Ve,Ktrans,angiopoietin 2,and Kep were increased in stages Ⅲ and Ⅳ;compared with stage Ⅲ,vascular endothelial growth factor,Ve,Ktrans,and angiopoietin 2 in stage Ⅳ,Kep were increased significantly(P<0.05).Pearson correlation analysis showed that Ve,Ktrans,and Kep were positively correlated with angiopoietin 2 and vascular endothelial growth factor(P<0.05).The ROC curves indicated that the predictive efficacy of the combined test of Ve,Ktrans,and Kep was higher than that of the single prediction(P<0.05).Conclusion:Ve,Ktrans,and Kep are increased in breast cancer patients and positively correlated with angiopoietin-2 and vascular endothelial growth factor levels.Concurrently,the combined detection of Ve,Ktrans,and Kep demonstrates high predictive efficacy for breast cancer.Clinically,DCE-MRI examinations of breast cancer patients can provide reference for clinical preliminary assessment of disease status and subsequent treatment.
