颅脑超声与肌酸激酶同工酶联合检测在新生儿颅内出血诊断中的应用价值研究OA
Research on the application value of combined detection of cranial ultrasound and creatine kinase isoenzyme in the diagnosis of intracranial hemorrhage in neonates
目的:探讨颅脑超声(CUS)与肌酸激酶同工酶(CK-MB)指标联合检测在新生儿颅内出血(ICH)诊断中的应用价值.方法:回顾性纳入 2020 年 1 月—2024 年 12 月我院新生儿科收治的经CT/MRI检查确诊为ICH的 42 例患儿作为观察组,另选同期 41 例临床排除ICH患儿作为对照组.两组患者均接受CUS、CK-MB检测.比较两组新生儿大脑中动脉参数、CK-MB;比较观察组CUS、CK-MB检测及联合检测对ICH患者的检出例数、检出率和诊断效能.结果:CUS诊断 33 例,漏诊 9 例,CK-MB诊断 31 例,漏诊 11 例.与对照组患儿比较,观察组患儿大脑中动脉收缩期峰值流速(PSV)参数较低、阻力指数(RI)参数和CK-MB较高,差异均有统计学意义(P<0.05).观察组CUS、CK-MB两者联合检测的检出率为 85.71%,均高于CUS检出率(78.57%)、CK-MB检出率(73.81%),但无显著性差异(P<0.05).与CUS、CK-MB单项诊断相比,联合检测对ICH的诊断价值较高,差异有统计学意义(P<0.05).联合检测对ICH的曲线下面积最大,为 0.937,灵敏度为 0.857.结论:CUS与CK-MB联合检测在ICH诊断中的诊断效能显著优于单项检测,能够为早期诊断及干预提供依据.
Objective:To explore the application value of combined detection of cranial ultrasound(CUS)and creatine ki-nase isoenzyme(CK-MB)in the diagnosis of intracranial hemorrhage(ICH)in neonates.Methods:It included 42 neonates diag-nosed with ICH through CT/MRI examination in our hospital from January 2020 to December 2024 as the observation group retrospectively.Another 41 neonates without ICH in our hospital during the same period were selected as the control group.Both groups underwent CUS and CK-MB tests.The parameters of the middle cerebral artery and CK-MB levels were com-pared between two groups of newborns,and the number of detected cases,detection rates,and diagnostic efficacy of CUS,CK-MB test,and combined test for ICH patients were compared in the observation group.Results:The number of cases di-agnosed by CUS was 33,with 9 missed diagnoses.The number of cases diagnosed by CK-MB was 31,with 11 missed diag-noses.Compared with the control group,the observation group had lower peak systolic velocity(PSV)parameters and higher re-sistance index(RI)parameters and CK-MB levels,and the differences were statistically significant(P<0.05).The detection rate of the combined CUS and CK-MB test in the observation group was 85.71%,which was higher than the detection rate of CUS(78.57%)and CK-MB(73.81%),but there was no significant difference(P<0.05).Compared with the single diagnosis of CUS and CK-MB,the combined test had a higher diagnostic value for ICH,and the difference was statistically significant(P<0.05).The combined test had the largest area under the curve(AUC)for ICH,which was 0.937,and the sensitivity was 0.857.Con-clusion:The diagnostic efficacy of combined detection of CUS and CK-MB in ICH diagnosis is significantly superior to that of single-item detection.It can provide a basis for early diagnosis and intervention.
