目的 通过对磷脂酰肌醇3-激酶/蛋白激酶B/诱导型一氧化氮合酶(PI3K/AKT/iNOS)通路轴的研究,探讨电针对豚鼠干眼的治疗作用机制.方法 将24只英国短毛三色豚鼠随机分为空白组、模型组、西药组和电针组,每组6只.空白组不做任何处理,其他组为氢溴酸东莨菪碱皮下注射35 d诱导的干眼模型.西药组双侧给予氟米龙滴眼液滴双眼干预,电针组给予双侧睛明、太阳、攒竹、瞳子髎和丝竹空穴电针干预,西药组与电针组均干预14 d.干预后0 d、22 d和35 d检测各组豚鼠泪膜破裂时间(BUT)、角膜荧光素染色(FL)和泪液分泌量(SIT).光镜下观察豚鼠泪腺与结膜组织苏木精-伊红(HE)染色形态,利用积分光密度法分析结膜组织炎性细胞浸润程度.Western blot法检测豚鼠泪腺中PI3K、AKT、磷酸化AKT(p-AKT)、诱导型一氧化氮合酶(iNOS)蛋白表达水平.qRT-PCR法检测豚鼠泪腺iNOS、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)的mRNA相对表达量.结果 干预后35 d,与空白组相比,模型组豚鼠BUT下降,FL评分上升,SIT减少,差异均有统计学意义(均为P<0.05);与模型组相比,西药组和电针组豚鼠BUT上升,FL评分下降,SIT增加,差异均有统计学意义(均为P<0.05).光镜下,与空白组相比,模型组豚鼠泪腺与结膜组织出现明显病理改变;与模型组相比,西药组和电针组豚鼠泪腺、结膜组织病变改善.积分光密度分析结果显示,与空白组相比,模型组豚鼠结膜组织炎性细胞浸润程度增加,差异有统计学意义(P<0.05).与模型组相比,电针组豚鼠结膜组织炎性细胞浸润程度减少,差异有统计学意义(P<0.05).Western blot检测结果显示,与空白组相比,模型组豚鼠泪腺组织PI3K、p-AKT/AKT、iNOS蛋白相对表达量均显著上升,iN-OS、IL-6、TNF-α的mRNA相对表达量均显著上升,差异均有统计学意义(均为P<0.05);与模型组相比,西药组和电针组豚鼠泪腺组织PI3K、p-AKT/AKT、iNOS蛋白相对表达量均显著下降,iNOS、IL-6、TNF-α的mRNA相对表达量均显著下降,差异均有统计学意义(均为P<0.05).结论 电针可改善干眼模型豚鼠泪腺炎症反应,抑制PI3K/AKT通路过度激活,降低下游iNOS、IL-6、TNF-α等炎症标志物转录水平.

Objective To investigate the therapeutic mechanism of electroacupuncture in dry eye disease in guinea pigs through the phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT)/inducible nitric oxide synthase(iNOS)pathway axis.Methods Twenty-four British short-haired tricolor guinea pigs were randomly assigned to the blank group,model group,western medicine group,and electroacupuncture group,with 6 guinea pigs in each group.The blank group re-ceived no treatment,while dry eye models were induced in the other groups by subcutaneous injection of scopolamine hydrobromide for 35 consecutive days.The western medicine group was treated with fluorometholone eye drops bilaterally,and the electroacupuncture group received electroacupuncture at bilateral acupoints including Jingming,Taiyang,Cuanzhu,Tongziliao,and Sizhukong.Both the western medicine group and the electroacupuncture group were treated for 14 consecu-tive days.Tear film break-up time(BUT),corneal fluorescein staining(FL),and Schirmer I test(SIT)were measured at day 0,day 22,and day 35 after intervention.Hematoxylin-eosin staining was used to evaluate the histomorphology of the lac-rimal gland and conjunctival tissues under light microscopy.The degree of inflammatory cell infiltration in the conjunctival tissue was analyzed using the integral optical density method.Western blot was performed to detect the protein expression of PI3K,AKT,phosphorylated AKT(p-AKT),and iNOS in the lacrimal gland of guinea pigs.Quantitative real-time polymer-ase chain reaction was employed to measure the relative mRNA expression of iNOS,interleukin-6(IL-6),and tumor necro-sis factor-α(TNF-α).Results At day 35 after intervention,the model group exhibited shortened BUT,elevated FL score,and reduced SIT compared with the blank group,and the differences were statistically significant(all P<0.05).The west-ern medicine group and the electroacupuncture group showed prolonged BUT,decreased FL score,and increased SIT rela-tive to the model group,and the differences were statistically significant(all P<0.05).Under light microscopy,the model group displayed marked histopathological changes in the lacrimal gland and conjunctiva,compared with the blank group.Compared with the model group,the western medicine group and the electroacupuncture group demonstrated alleviated tis-sue injury in the lacrimal gland and conjunctiva.The results of integral optical density analysis showed that compared with the blank group,the inflammatory cell infiltration in the conjunctival tissue of guinea pigs in the model group was aggrava-ted,and the difference was statistically significant(P<0.05).Compared with the model group,the inflammatory cell infil-tration in the conjunctival tissue of guinea pigs in the electroacupuncture group was alleviated,and the difference was statis-tically significant(P<0.05).According to Western blot results,lacrimal gland tissues from the model group showed signifi-cantly upregulated protein expression of PI3K,p-AKT/AKT,and iNOS,alongside elevated mRNA levels of iNOS,IL-6,and TNF-α,compared with the blank group,and the differences were statistically significant(all P<0.05).The western medi-cine group and the electroacupuncture group exhibited downregulated PI3K,p-AKT/AKT,and iNOS protein levels,as well as reduced mRNA expression of iNOS,IL-6,and TNF-α,relative to the model group,and the differences were statistically signif-icant(all P<0.05).Conclusion Electroacupuncture mitigates lacrimal gland inflammation in guinea pigs with dry eye,suppresses the overactivation of the PI3K/AKT pathway,and lowers the transcriptional levels of downstream inflammatory markers,including iNOS,IL-6,and TNF-α.
