目的:探讨槲皮素对膝骨性关节炎(KOA)大鼠滑膜炎症的作用及机制.方法:建立KOA模型大鼠,随机分为假手术组、模型组、槲皮素低剂量组(50 mg/kg)、槲皮素高剂量组(100 mg/kg)和塞来昔布组(20 mg/kg),每组10只.各组大鼠每日灌胃相应药物1次,持续4周.检测各组大鼠关节痛行为学;HE染色观察关节组织病理变化;酶联免疫吸附试验(ELISA)检测血清和关节液白细胞介素(IL)-6、单核细胞趋化蛋白-1(MCP-1)、IL-10水平;免疫组化染色检测滑膜组织肿瘤坏死因子-α(TNF-α)、环氧化酶-2(COX-2)水平;Western blot检测滑膜组织诱导型一氧化氮合酶(iNOS)、CD68、精氨酸酶1(Arg1)、CD206蛋白表达.结果:与假手术组比较,模型组大鼠机械痛阈值、热敏痛阈值降低(均P<0.05);血清和关节液IL-6、MCP-1水平升高,IL-10水平降低(均P<0.05);膝关节组织中软骨退变伴滑膜炎性增生,且滑膜组织TNF-α、COX-2、iNOS、CD68蛋白水平升高,Arg1、CD206蛋白水平降低(均P<0.05).与假手术组比较,槲皮素低、高剂量组和塞来昔布组大鼠机械痛阈值、热敏痛阈值升高(均P<0.05);血清和关节液IL-6、MCP-1水平降低,IL-10水平升高(均P<0.05);膝关节组织病理损伤减轻,且滑膜组织TNF-α、COX-2、iNOS、CD68蛋白水平降低,Arg1、CD206蛋白水平升高(均P<0.05).结论:槲皮素可缓解KOA大鼠关节疼痛症状,减轻其滑膜炎症,其机制可能与促进M2型巨噬细胞极化有关.

Objective:To investigate the effects and mechanism of quercetin on synovial inflammation in rats with knee osteoarthritis(KOA).Methods:A rat model of KOA was established and randomly divided into the fol-lowing groups(n=10 each):sham-operated group,model group,low-dose quercetin group(50 mg/kg),high-dose quercetin group(100 mg/kg),and celecoxib group(20 mg/kg).All groups received daily intragastric administration of the respective agents for 4 weeks.Behavioral assessments of joint pain were conducted.Histopathological altera-tions in joint tissues were visualized via HE staining.Interleukin-6(IL-6),monocyte chemoattractant protein-1(MCP-1),and IL-10 concentrations in serum and synovial fluid were quantified by ELISA.Synovial protein expres-sion was profiled using immunohistochemistry and Western blot.Specifically,immunohistochemistry was utilized for the detection of tumor necrosis factor-α(TNF-α)and cyclooxygenase-2(COX-2),whereas Western blot was used for the quantification of inducible nitric oxide synthase(iNOS),CD68,arginase1(Arg1),and CD206.Results:Compared with the sham group,KO A model rats exhibited decreased mechanical and thermal pain thresholds(all P<0.05);IL-6 and MCP-1 levels were elevated and IL-10 level was reduced in serum and synovial fluid(all P<0.05);cartilage degeneration was accompanied by synovial inflammatory hyperplasia;synovial expression of TNF-α,COX-2,iNOS,and CD68 proteins were increased,along with decreased Arg1 and CD206 proteins(all P<0.05).Relative to the model group,both low-and high-dose quercetin groups and celecoxib group reversed these changes:mechanical and thermal pain thresholds were increased(all P<0.05));IL-6 and MCP-1 levels in serum and synovial fluid were de-creased while IL-10 level was increased(all P<0.05);histopathological damage was alleviated;and TNF-α,COX-2,iNOS,and CD68 protein levels in synovial were reduced,whereas Arg1 and CD206 protein levels were elevated(all P<0.05).Conclusion:Quercetin alleviates joint pain and attenuates synovial inflammation in KOA rats,possibly by promoting M2 macrophage polarization.
