急性心肌梗死病人多维疲劳与运动感知的相关性OA
Correlation between multidimensional fatigue and exercise perception in patients with acute myocardial infarction
目的:探讨急性心肌梗死病人不同时间点的多维疲劳与运动感知情况,并构建交叉滞后模型,分析多维疲劳与运动感知的动态关联作用.方法:选取2023年5月—2025年1月我院收治的急性心肌梗死病人308例.采用一般资料调查表、中文版运动感知量表(ESQ)、中文版多维疲劳量表(MFI-20)调查病人出院前1 d、出院后1个月及3个月的多维疲劳与运动感知水平.结果:出院前1 d、出院后1个月及3个月时,急性心肌梗死病人MFI-20评分依次为(57.12±6.42)分、(51.69±4.93)分、(42.39±5.07)分,ESQ评分依次为(40.24±5.68)分、(36.57±4.74)分、(29.13±4.16)分.不同美国纽约心脏病学会(NYHA)分级、合并症的急性心肌梗死病人3个时间点MFI-20与ESQ评分比较差异均有统计学意义(均P<0.05).Pearson相关分析显示,3个时间点MFI-20与ESQ评分之间均呈正相关(均P<0.05).交叉滞后模型结果显示模型拟合效果较好.出院前1 d多维疲劳可正向预测出院后1个月运动感知(P<0.01),出院后1个月多维疲劳可正向预测出院后3个月运动感知(P<0.01);出院前1 d运动感知也可正向预测出院后1个月多维疲劳(P<0.01),出院后1个月运动感知可正向预测出院后3个月多维疲劳(P<0.01).结论:急性心肌梗死病人出院后3个月内多维疲劳与运动感知水平呈现下降趋势,但多维疲劳与运动感知互为因果,二者存在动态交互影响关系.临床护理人员需同时关注疲劳缓解与运动感知改善的协同干预,促进病人康复.
Objective:To explore the multidimensional fatigue and motor perception of patients with acute myocardial infarction at different time points,and to construct a cross-lag model to analyze the dynamic correlation between multi-dimensional fatigue and motor perception.Methods:A total of 308 patients with acute myocardial infarction admitted to our hospital from May 2023 to January 2025 were selected.The general information questionnaire,the Chinese version of the Exercise Sensitivity Qustionnaire(ESQ),and the Chinese version of the Multidimensional Fatigue Inventory(MFI-20)were used to investigate the levels of multidimensional fatigue and motor perception of the patients 1 day before discharge,1 month after discharge,and 3 months after discharge.Results:One day before discharge,1 month after discharge,and 3 months after discharge,the MFI-20 scores of patients with acute myocardial infarction were 57.12±6.42,51.69±4.93,and 42.39±5.07 respectively.The ESQ scores were 40.24±5.68,36.57±4.74,and 29.13±4.16 respectively.There were statistically significant differences in terms of MFI-20 and ESQ scores among patients with different NYHA classifications and comorbidities at the three time points(all P<0.05).Pearson correlation analysis showed that there was a positive correlation between MFI-20 and ESQ scores at the three time points(all P<0.01).The results of the cross-lag model showed that the model had a good fitting effect.The multidimensional fatigue of patients 1 day before discharge could positively predict their movement perception 1 month after discharge(P<0.01).The multi-dimensional fatigue of patients 1 month after discharge could positively predict their movement perception 3 months after discharge(P<0.01).The movement perception of patients 1 day before discharge could also positively predict the multi-dimensional fatigue 1 month after discharge(P<0.05).The movement perception of patients 1 month after discharge could also positively predict the multi-dimensional fatigue of them 3 months after discharge(P<0.01).Conclusions:Within 3 months after discharge,multidimensional fatigue and the level of movement perception in patients with acute myocardial infarction showed a downward trend.However,multidimensional fatigue and movement perception were mutually causal.There was a dynamic interactive influence relationship between them.Clinical nurses should pay attention to the collaborative intervention of fatigue relief and improvement of movement perception,so as to promote the recovery of patients.
